The premiere episode of "4Minutes" kicks off with Dr. Khomsan engaged in a desperate struggle to resuscitate a patient teetering on the brink of death. Meanwhile, Tyme, drenched in his own blood, attempts to flee but is intercepted by an armed assailant who fires a shot, bringing an abrupt halt to his escape.

Elsewhere, Great, a university student in his final year, prepares to embark on his exams. As he steps into the elevator, a panic attack strikes him unexpectedly. Upon emerging, he manages to compose himself, revealing through his luxury car that he belongs to the affluent Sriwat family. His father, a stern patriarch, insists that his children follow in his footsteps and join the family enterprise. Consequently, Korn, Great's elder brother, has been entrusted with overseeing the "investment" department, a euphemism for an illegal gambling ring. Mrs. Sriwat, equally influential, harbors ambitions for Great to inherit Korn's former role, though he rarely finds himself at home.

Great's journey to university is interrupted by a call from his friend, Title, who urgently requests an extra calculator for their exams. In his haste to locate one, Great becomes late and decides to speed, ultimately leading to a collision with a pedestrian. Frightened, he initially flees the scene but is inexplicably transported back in time to the moment Title's call first rang out.

With a newfound sense of confusion, Great this time opts to drive cautiously. We witness the woman deliberately stepping into his path, and although he still strikes her, the impact is significantly mitigated. Despite her pleas to the contrary, he insists on calling for assistance.

At the hospital, we are introduced to Dr. Den, a whimsical and charismatic figure who frequently clashes with Nurse Bee, a deeply superstitious individual. His attempts to attend to other matters are halted by the arrival of the accident victim, Mrs. Manee, who lacks both funds and the desire for treatment. Tyme, one of the attending surgeons, approaches the situation with clinical detachment, duty-bound to assist her. However, Den employs his charm and persuasiveness to convince her to undergo surgery, despite her initial resistance.

During the surgery, it becomes evident that this is Tyme's maiden operation, revealing his status as a trainee. His mentor, Dr. Khomsan, expresses his intention for Tyme to serve as an instructor upon completing his training, but Tyme firmly expresses his desire to work in a private hospital instead. While he demonstrates efficiency in his tasks, his demeanor remains distant, to the point that he can't even recall Manee's name. In stark contrast, Den finds no difficulty in engaging Tyme in conversation, who readily opens up to him. We learn that Den is conducting research on patients who have experienced cardiac arrests. Additionally, Tyme receives calls from someone whom Den assumes to be his girlfriend.

Elsewhere, at the gambling headquarters, the veteran boss, Sa-marn, is showing Korn the shareholder information. Suddenly, a girl with shifty eyes, one of the laborers, intrudes, using the pretext of delivering water. Sa-marn promptly ejects her, but not before she takes a mental note of the layout.

Korn, overwhelmed by these recent developments, seeks solace in his boyfriend, Tonkla, a freelancer who steadfastly insists on working despite Korn's generosity. Tonkla also conceals his smoking habit from Korn. As they share an intimate moment, they are interrupted by Korn's father, who summons him home. Korn hastily prepares to leave, resorting to lies about his whereabouts, leaving Tonkla visibly disappointed. Upon Korn's departure, Tonkla encounters a black cat, which visibly shakes him.

At home, the atmosphere during dinner was thick with tension as Mr. Sriwat lambasted Great for the mishap. His wife, appearing more benevolent, suggested that Great send flowers to Manee. However, she cautiously added that he should snap a picture of his gesture of goodwill as proof, just in case anyone attempted to sue or tarnish their reputation. Korn escorted Great home, where the latter was beset with panic as he noticed his watch was far ahead of the actual time. Korn stayed with him, suspecting that Great might be under the influence of drugs, and displayed his aversion to cigarettes by repeatedly crushing the ones Great attempted to smoke.

As they conversed, Great confessed to Korn that he had a glimpse of the future before colliding with Manee. Korn, skeptical, dismissed it as exam-induced stress. Despite their different mothers, it was evident that the two shared a close bond. Meanwhile, Tyme headed home, his financial situation precarious. His grandmother, battling diabetes, ran a dessert business to keep their heads above water. His sole ambition was to become a wealthy surgeon so he could care for her.

The following day, Den's research was in high gear. He met with patients who recounted their out-of-body experiences during their cardiac arrests. In the meantime, Great visited the hospital, puzzled that he hadn't experienced an attack in the elevator. He attempted to leave flowers for Manee, but she intercepted him, reassuring him that he wouldn't face consequences since she deliberately caused the accident. Great, flustered, hastily exited and accidentally bumped into Tyme, who tumbled to the ground. Tyme, irate, called out to him, leaving Great disoriented. Suddenly, he was transported back four minutes, to the moment he inquired about Manee's room number from a nurse.

More perplexed, he deposited the flowers at the reception and noticed Tyme. He had a vision of them kissing, but this time, Tyme didn't collide with him; instead, he visited a patient. Deep in thought, Great decided to depart, but Tyme emerged from the patient's room just then. As fate would have it, they bumped into each other again, causing Tyme's patient files to spill. Great hastily tried to assist him. At that precise moment, Den interviewed Ms. Lukwa, who revealed that during her cardiac arrest, her heart stopped, and she traveled four minutes into the future. With that intriguing revelation, the recap of 4Minutes episode 1 drew to a close.
