His destination? The ominous Blackfield Asylum – a place where Sweet Tooth harbors a personal vendetta, eager to settle an old score.
Quiet and John arrive at a secluded rest stop, Nuthouz, a haven where the elite Milkmen often pause for sustenance and rejuvenation. Amongst its patrons, they unexpectedly encounter Mary, John's former love whom he had cruelly abandoned. Mary's reputation precedes her; Mr. Slam, the affable bartender, divulges that she is among the most renowned Milkmen of all time.
A shocking revelation follows: Marcus, their acquaintance, is none other than the infamous Sweet Tooth. Following a catastrophic event, Marcus was confined to an asylum for a decade, where he amassed an unsavory cadre of "companions." Stu, Marcus's current companion, learns the harrowing truth that the co-star he had inadvertently slain was but a canine, and that Marcus had imprisoned his own parents in a cell, where they ultimately perished, victims of both neglect and an insidious pestilence.
