Marcus Kane, the erstwhile star of the show, harbors a bitter discontent over the titular addition, his resentment simmering beneath the surface.

As the cameras roll, an unexpected usurper steals the spotlight – none other than Billy, a radiant golden retriever, whose charm captivates all. Enraged beyond reason, Marcus snaps, seizing a pair of sharp sewing needles and, in a fit of madness, ends Billy's innocent life before a horrified audience.

Fast-forwarding to the present, the narrative shifts to John and Quiet, who have arrived at the gates, their mission to retrieve a mysterious package now within reach. Yet, an obstacle looms in the form of a treacherous moat, guarding the entrance to their destination. With a strategic nudge of a stone, the portal to the building reluctantly yields, revealing a foreboding hallway. A haunting voice echoes through the corridor, leading them on a guided tour, and John's intuition whispers the name Calypso, a legendary figure in these parts.

Securing the package, the duo hastily departs, hitting the open road once more. To evade the treacherous path through Topeka, they embark on a longer, more circuitous journey, choosing to traverse the treacherous North.

Meanwhile, Agent Stone's radio crackles with urgent news from Carl, revealing that Sweet Tooth has unleashed a reign of terror, striking three more outposts and making off with all prisoners in tow. His destination? The ominous Blackfield Asylum – a place where Sweet Tooth harbors a personal vendetta, eager to settle an old score.

Quiet and John arrive at a secluded rest stop, Nuthouz, a haven where the elite Milkmen often pause for sustenance and rejuvenation. Amongst its patrons, they unexpectedly encounter Mary, John's former love whom he had cruelly abandoned. Mary's reputation precedes her; Mr. Slam, the affable bartender, divulges that she is among the most renowned Milkmen of all time.

A shocking revelation follows: Marcus, their acquaintance, is none other than the infamous Sweet Tooth. Following a catastrophic event, Marcus was confined to an asylum for a decade, where he amassed an unsavory cadre of "companions." Stu, Marcus's current companion, learns the harrowing truth that the co-star he had inadvertently slain was but a canine, and that Marcus had imprisoned his own parents in a cell, where they ultimately perished, victims of both neglect and an insidious pestilence. Stu is visibly disturbed yet endeavors not to disturb Marcus's fragile equilibrium.

Marcus, overcome with emotion, confides to the silent witnesses of his past deeds, the skeletal remains of the asylum, that he had never aspired to the world of sitcoms. It was his parents' relentless ambition, fueled by the promise of wealth, that had coerced him into this unwanted path.

As Marcus and Stu venture back into the courtyard, they are confronted with a grisly scene: Stone and his ruthless officers have carried out a massacre, leaving no prisoner alive. They fire upon the duo, assuming them to be among the fallen. Yet, amidst the chaos, Mike, one of the shooters, deliberately misses, his aim faltering.

Back at Nuthouz, the atmosphere is charged as John divulges to Mary that this mission marks the end of his journey. He whispers of New San Francisco, painting a tantalizing picture that captivates the ears of the Milkmen gathered around. John cunningly exploits this moment, querying aloud, "To whom shall I entrust this package?" The question ignites a fierce brawl among the ravenous Milkmen, Mary's loyalists even turning against John and Quiet in their desperation.

It all culminates in a tense confrontation between Quiet and Mary, with Quiet's blade pressed perilously close to Mary's throat. In this moment of truth, Mary's vulnerability is exposed as she confesses her jealousy, stemming from John's failure to invite her along on his journey. Quiet, however, demonstrates mercy, her voice laden with compassion as she warns, "Do not let the quest for revenge consume your chance at redemption." But Mary remains unyielding, her defiance igniting a final, tragic clash as Quiet's blade pierces her chest.

Meanwhile, at the asylum, Stu's heroics shine once more, as he masterfully manipulates Marcus' mind, convincing him that Mike holds the key to leading them to Stone. The episode reaches a pivotal turning point with Quiet and John's relationship undergoing a remarkable transformation. John, in a gesture of profound trust, entrusts Quiet with the car keys, symbolizing their growing bond as they embark on their journey together.

As they depart, the stage is subtly set for a new twist, with Pit Viper, a shadowy Milkman from the bar, slyly slipping Stone a text message, divulging the duo's destination. This clandestine communication hints at unseen forces at play, weaving an intricate web of intrigue that promises to keep viewers on the edge of their seats.
