Marlene hypothesizes that Thunder, with an uncanny intuition, senses Owen's tension stemming from a hidden darkness. Owen's mind drifts back to the tragic loss of his brother, yet he keeps silent.

Elsewhere, Wyatt finds himself in a dramatic scenario, assisting a handcuffed man trapped inside a blazing police van. Fortunately, the prisoner, instead of fleeing, heroically rescues an unconscious policeman from his burning vehicle.

At the fire station, an awkward mishap occurs when Nancy, intending to share a snap of the mouthwatering chicken breast Matteo had prepared for dinner, inadvertently sends a picture of her breasts instead. In a panic, she and Matteo scramble to delete the incriminating image from everyone's phones before it's noticed. However, Tommy catches a glimpse and requests a private conversation with Nancy. Fearful of HR's reprisal, Nancy is relieved when Tommy's concern stems from noticing a lump on her breast. He urges her to see a doctor and grants her the rest of the night off.

Later, Nancy confides in Matteo about her terror, revealing that breast cancer claimed the lives of her great-grandmother, grandmother, and mother. Matteo, taken aback by her silence and lack of preventative measures, questions her. Nancy feels accused, and Matteo quickly apologizes. As she leaves, he vows to keep in touch via text.

Regrettably, post the conclusion of their work shift, Nancy persists in disregarding Matteo's messages. Rather than delving into specifics, Matteo seeks counsel from Marjan, who urges him to exercise patience, hinting that Nancy might be seeking solace elsewhere. In response, Matteo enlists Tommy's help to accompany Nancy to her appointment. Tommy, agreeable to the task, catches Nancy off guard at the hospital. Pretending to be there for a mammogram, Nancy, however, is acutely aware of Matteo's involvement through Tommy. She confides in Tommy, revealing her anxiety about her upcoming tests. Her mother, diagnosed just two months prior, succumbed swiftly to her illness, and Nancy's memories of her mother's ordeal are still vivid. She fears that Matteo might witness her in a similar vulnerable state, a fate she is desperate to avoid. Tommy argues that Matteo is capable of handling the situation, but Nancy remains uncertain of her own strength.

Meanwhile, TK is troubled by Owen's behavior following a sleepwalking episode during their night shift. Owen repeatedly apologized, murmuring, "I am sorry, I promised," and narrowly missed injuring Matteo. TK suspects that his father is grappling with something profound, as such occurrences only manifest during times of severe distress. Owen claims to be worried about Thunder, but TK is unconvinced.

The following day, the medical team is summoned to rescue a man trapped under a vending machine. The victim had spent the entire day with his hand stuck, eventually freeing it but subsequently being crushed by the toppling machine. Sadly, due to his delay in seeking help, the team's efforts were largely in vain. This harrowing incident serves as a wake-up call for Nancy, realizing that she cannot let fear of seeking assistance dictate her life. She subsequently apologizes to Matteo, who reassuringly vows to stand by her as the doctors convey her test results.

Elsewhere, Owen finally unburdens himself to Thunder, revealing the secret that has been consuming him from within. He lays the blame squarely on his own shoulders for Robert's untimely demise. As soon as the words escape his lips, Owen's mind begins to wander, and he succumbs to vivid hallucinations, eventually losing consciousness. Marlene discovers him in this distressed state and swiftly administers first aid.

Upon regaining his senses, Owen realizes that his fainting spell was triggered by a gas leak emanating from the barn. In a rush to escape, they find the barn already engulfed in flames. Without hesitation, Owen instructs Marlene to shut off the gas line while he bravely ventures inside to rescue the horses and workers trapped within. Miraculously, he manages to save everyone just in the nick of time, earning Thunder's unconditional trust as they flee the inferno together.

A couple of days later, Owen returns to lend a helping hand to Marlene in constructing a new barn. Alongside his friends and colleagues, he puts in tireless effort, their shared determination visible in every plank of wood and nail they hammer into place.

The episode concludes with Nancy visiting Tommy to deliver the reassuring news that her diagnosis was a false alarm—merely a cyst, not cancer. However, Tommy's mood darkens as he reveals that the doctors contacted him after Nancy's mammogram, revealing a grim discovery. The news, he says, is dire and heart-wrenching.
