In the opening episode of "Dear Killer Nannies," Juan, Pablo Escobar's son, takes us through his life story, describing how his father's profession had a profound and negative impact on him, both as a child and as he grew into adulthood. One fateful day, the men hit a deer with their car while driving under the influence. Juan notices that the deer is still alive and begs them to call a vet. The men refuse and hand him a gun, asking him to end the animal's misery. Juan panics, but a young woman named Angie steps in and kills the deer for him.

The group then takes Juan back to The Hacienda, an estate built by his father. Back at the estate, Juan approaches El Dorado, the family's bodyguard, and asks to see Pablo. El Dorado tells him that Pablo is busy at work.

Meanwhile, Juan's mother, Victoria, notices the scars on his face and asks about them. Juan lies to protect the men - Lagaña, Mohawk Nanny, and Tina - out of fear of what they might do to him.

At dinner, Juan asks about his father, who is not present. Victoria speaks about Pablo's kindness towards the men, while Juan teasingly calls them his "nannies."

Later that night, Juan ventures into the game room where the nannies are smoking, doing drugs, and spending time with women. He asks a nanny named Kiss to play with him. The episode concludes with Juan's complex relationship with these men who, despite their criminal ways, hold a special place in his heart due to their closeness with his father.

As the night deepens, El Dorado urges Juan to turn in for the night, but his wanderlust leads him instead to Pablo's office. The narrative delves into how Pablo's political ambitions drive a wedge between him and his family. Juan recalls the difficulty of making friends at school due to the armed guards that shadow him. He often visits Pablo's office to steal fleeting moments with his father amidst his hectic workday.

During one such visit, Juan meets Rodrigo, a 17-year-old boy brought by his mother to seek work from Pablo. Juan later confides that Rodrigo becomes his closest companion. Rodrigo is swiftly initiated into the group of deadly nannies who protect Juan and the Escobar family.

Meanwhile, Juan hears news of the Minister of Justice claiming to have evidence against Pablo. His concern for his father's safety mounts, but Victoria tries to quell his fears. In private, Juan tells Rodrigo that he will kill anyone who threatens his father. Soon after, Juan unwittingly assists the nannies in trapping a man they've been searching for.

The nannies' brutal treatment of the man, including their beatings, terrifies Juan. Later that night, he witnesses Lagaña and Tina torture the man while Pablo burns him alive, with Rodrigo trying to comfort Juan.

The episode then shifts to 1993, where Juan is now a teenager. With people vying to eliminate Pablo's family, Victoria attempts to send Juan, his sister Manuela, and his girlfriend Andrea out of Colombia. At the airport, the staff alerts the police, and Juan, Andrea, and Manuela are denied permission to leave the country. Fearing for their safety, Juan enlists his cousin to bring the press and media to the airport.

One staff member allows Juan to take a helicopter out, but the trio is left on their own. While hiding, Pablo calls Victoria during a shootout. He urges Juan to make a public statement accepting his guilt and requesting police protection for the family from their enemies. The call is cut short by gunfire. Later, news breaks of Pablo's death, and Juan and Victoria watch the report from their hotel as the episode comes to a close.
