Episode 4 of Grey's Anatomy's 20th season kicks off with Kwan and the fellow interns under scrutiny for their decision to operate on Sam. The looming threat of a colossal lawsuit hangs heavy over Grey Sloan, prompting rigorous preparations. Each intern, from Kwan to Lucas, confesses that Sam's fate would have been grim regardless of their intervention. However, Simone reveals to the lawyers that she would never have initiated the surgery.

Dorian awakens in the ICU, receiving a briefing from Ndugu and Kwan on his condition. Later, Schmitt requests Kwan to insert a subclavian line, but during the delicate procedure, Dorian complains of excruciating chest pain, and his blood pressure plummets. Schmitt swiftly intervenes, inserting a chest tube to aid in re-inflating his lung, and Dorian's breathing resumes. Kwan acknowledges his error, confessing that he froze in the moment, haunted by the image of Maxine.

As the episode unfolds, Dr. Arizona arrives at Grey Sloan, ready to perform an unprecedented in-utero brain surgery, a daring feat where doctors delve into a baby's brain before birth. This procedure is fraught with risk, and the chances of success are uncertain, leaving the baby vulnerable to either mortality or permanent brain damage. Yet, Arizona and Bailey are resolute in their conviction.

The plot thickens when Vita, the mother, vacillates and decides against the surgery. She finds the prospect of doctors probing her unborn child's brain unacceptable. Vita demands to be discharged, a decision fraught with peril as the baby's survival hinges on the surgery. Moreover, if Vita reverses her decision later, it might be too late. However, Vita's husband implores Arizona to continue preparing and strive to convince Vita.

Finally, Vita relents and agrees to the surgery. Arizona succeeds in persuading Bailey to allow the interns into the operating theater. Despite minor setbacks, the operation culminates in success, and joyous tears are shed by all present.

Joshy, a loyal subscriber of Millin's brother, arrives at the hospital, his face etched with pain as he complains of a nagging backache. Upon examination, it is diagnosed as an abscess, requiring immediate attention. Millin attempts to drain it, but to no avail. Frustrated, she seeks the assistance of Yasuda, who offers her aid with a mischievous twist—she insists on peppering Millin with questions about her childhood.

Despite their combined efforts, the abscess remains stubbornly resistant to drainage. In a moment of desperation, they call upon the expertise of Dr. Hunt, and together, they successfully extract the pus from the swollen area.

As the episode draws to a close, the air is thick with tension between Simone and Lucas. Lucas feels a sense of betrayal from Simone, and the atmosphere is heavy with unresolved emotions. Suddenly, Yasuda bursts into the room, her face beaming with excitement as she announces that she has completed her procedure logs and is eager to return to the operating room. Her joy is fleeting, however, as Bailey sternly informs her that nobody can reenter the OR until all procedure logs are completed. The mood shifts from elation to disappointment in a matter of seconds.
