Meanwhile, Eyewitness breaks into laughter as he sets everything and everyone around him ablaze, transforming the scene into a fiery chaos. Just as he turns to leave, a beam cracks and crashes down upon him, clearly an unplanned twist in his fiery scheme. Despite his desperate attempts to wriggle free, he remains trapped beneath the heavy debris.
Flash forward to the present, Eyewitness storms into Jae-yeong’s apartment, only to receive a call from the insurance agent confirming the successful payout. However, to his dismay, Dong-sik has cunningly altered the beneficiary to the church.
How did Ju-yeon manage to outmaneuver Eyewitness? The narrative shifts to a poignant flashback, where Jeong-min witnesses Ju-yeon confronting Eyewitness in a taxi, her eyes burning with intent. Later, she subtly confesses her intent to kill Jae-yeong to Jeong-min, only to backtrack immediately. Yet, Jeong-min remains unconvinced, his face etched with concern.
Sure enough, Ju-yeon orchestrates a devious plan, paying Hwang to substitute Eyewitness's painkillers with Fentanyl. Hwang seizes the opportunity, bumping into Eyewitness in his haste to reach the church and stealthily switching the pills. Ju-yeon tails Eyewitness to the church, where he unleashes a torrent of anger on the nun. Spotting Ju-yeon, he unleashes a tide of vitriol at her. In a fit of rage, he consumes a few Fentanyl pills and storms off.
Undeterred, Ju-yeon continues to track him, and when he lunges at her, insisting he's not Jae-yeong, she refuses to believe him. As he stumbles and falls, she draws out a scalpel, her resolve hardening. Suddenly, Jeong-min appears, pleading with her not to descend into murder. Having uncovered the truth about Jae-yeong, he knows the gravity of her actions. Eyewitness taunts her, goading her on, but she hesitates, eventually giving up and leaving with Jeong-min, her scalpel clenched but unused.
In Jae-yeong's apartment, Eyewitness stumbles upon Ju-yeon's high school magazine cover, a discovery that serves as a catalyst for a haunting memory. A vivid flashback unfolds, depicting a youthful Yu-jeong brooding with resentment, determined to exact a lesson upon Ju-yeon. Fresh out of juvenile detention and having already graduated, Eyewitness finds himself in the midst of Yu-jeong's vengeful scheme. He suggests to her a dark plan—letting some of her male classmates ravage Ju-yeon.
Meanwhile, the police are engrossed in identifying the two corpses discovered in the desolate building. They are struck by an inconsistency: Eyewitness's claim that he was bound to a chair appears suspicious, given his discovery on the floor while one of the corpses was indeed tied up in such a manner.
Returning to his abode, Eyewitness finally comes to the realization that Ju-yeon had switched his pills. Just as this dawns upon him, Detective Kang rings to arrange another interrogation. Agreeing to meet the following day, Eyewitness comprehends that the law enforcement officers are hot on his trail. In a moment of clarity, he flees.
When the police arrive at his apartment, they find it empty but spot a crucial clue—a painkiller bottle bearing Eyewitness's fingerprints. They resolve to cross-reference these prints with the DNA evidence found in Jae-yeong's car, aiming to unequivocally establish his identity and involvement in the matter.
How does Jeong-min exact his punishment upon Eyewitness? As the alert signals D-Day, Eyewitness finds himself confronted by the timely arrival of the loan shark's henchmen, who promptly abduct him. Elsewhere, Jeong-min offers solace to Ju-yeon before proceeding to attend to his "ailing family member," a euphemism for his role as the doctor enlisted by the loan shark in the grim trade of organ trafficking. Driven by the need to eradicate his student debts and accumulate funds for his wedding, Jeong-min has taken on this desperate gig, with Eyewitness marking his final patient before he retires from such dark endeavors.
Upon encountering Eyewitness, mutual recognition dawns upon them, tinged with astonishment. Eyewitness finally confesses his true identity: not Park Jae-yeong, but Kim Beom-jun. However, Jeong-min remains unconvinced, suspecting a desperate lie conjured to escape the grim fate awaiting him. Embracing the notion of karma, Jeong-min takes a grim satisfaction in his task, proceeding to operate on Eyewitness without anesthesia, his blade slicing through flesh with a cold precision.
What transpires for the remaining characters? Ju-yeon awakens to find Jeong-min awaiting her, his words painting a picture of a brighter future ahead. PI Hwang arrives with newfound insights, yet Ju-yeon chooses to close that chapter of her life. With a serene smile, Hwang discards the DNA report, its contents unmistakably linking the fingerprints found on the painkillers to Kim Beom-jun, accompanied by photographs of Beom-jun, alias Eyewitness, and Park Jae-yeong.
As a macabre farewell gesture, the loan shark bestows upon Jeong-min the ill-gotten Rolex watch. As Karma Episode 6 draws to a close, Ju-yeon beams, her smile radiant as she steps out of the hospital after her shift, leaving behind the shadows of the past.
