In the fourth episode of Little Disasters, Jess's heart races as the doctors tend to Betsey, her intrusive thoughts once again painting a dire picture of the girl's fate. Thankfully, Liz descends to provide a reassuring update on Betsey's condition—a major seizure averted, thanks to their swift action. Despite Liz's assurance that Betsey will be fine, Jess harbors doubts, especially about trusting Liz with her child's health care.

As the couple retires to their home, another heated argument erupts. Ed urges Jess to stop exacerbating the situation, making clear that he empathizes with her pain. Mel, eavesdropping from her stay at their house, listens intently.
Meanwhile, Liz copes with the pressure and guilt by turning to alcohol. Charlotte, on the other hand, adopts a different tactic and reaches out to Luisa, the town's premier family lawyer. She pleads with her to intervene with Ed and outline his legal options if things take a turn for the worse. Charlotte reflects on their college nights out, subtly hinting at her crush on Ed and wondering if their relationship would have been different if he had known.
In an interview, Mel shares her suspicions about Charlotte and Ed's relationship dynamics. In contrast, Charlotte rants about how she is the perfect match for Ed, unlike Jess.
As Ed insists on leaving for work, Rob calls Mel to inquire about her return. Unfortunately, she can't leave yet. He blames her for leaving the kids with him, and after the call, Mel tries to help with the dishes, but Jess insists she is a guest. At Charlotte's house, Andrew reminds her about her upcoming egg retrieval appointment, offering to accompany her but she declines.
Elsewhere in the city, Jess embarks on a mission to secure Betsey's transfer to a private hospital, convinced that her current doctors are neglecting her needs. As the doctor at the new facility realizes the involvement of social workers in Betsey's case, she cautions Jessica against this move, her words laden with a sense of urgency.
At the other hospital, Mel takes Jess's children for their medical examination, her eyes trained on Frankie's arm where she discovers suspicious marks, as if he had been held too tightly. Seizing this opportunity, Mel turns to Liz for guidance, her mind racing with concerns that Jess might have caused these injuries. With each new discovery at Jess's home, Mel finds herself uncertain about what to report or say to Jess.
Meanwhile, Steve and Mollie pay a visit to Ed's office, questioning his reason for lying about going home for lunch on the day of Betsey's injury. Ed's defenses rise, explaining that he went back home to apologize to Jess after their earlier fight. He adds that Betsey was teething and agitated but otherwise perfectly safe. They delve deeper into his relationship with Charlotte, but Ed insists that he is not having an affair. A flashback reveals his decision to leave when Charlotte expressed a bittersweet nostalgia about their college days.
Sensing trouble brewing, Ed sends the police away, saying he will only speak to them in the presence of a lawyer. As the police depart, Ed calls Charlotte, who abruptly ends her egg retrieval appointment and rushes to Ed's aid.
At the social services office, Mel reports her findings to Lucy, determined to keep Jess's secret safe until she gathers more information. Lucy shares an incident from the hospital with the aim of swaying Mel's thoughts, adding a layer of intrigue to the already complex situation.
In the hospital, the weary and overworked Liz, nursing a hangover, grew increasingly grumpy. In her state of exhaustion, she deviated from protocol with one of their patients, only to be rebuked by Neil, who scolded her to slow down her alcohol consumption, fearing that Betsey's case had distracted her.
At school, Kit and Connor got into a heated argument after the latter bullied Frankie relentlessly. It turned out that Connor had been tormenting Frankie for quite some time. The teacher believed that Kit acted to protect his brother and that Connor was the culprit behind the wounds on Frankie's arm. He sent the brothers home with Mel and Jess, both of whom were embarrassed and apologetic on behalf of their son.
On the way home, Mel offered to buy coffee but was embarrassed when her card bounced. Meanwhile, Jess comforted Frankie, reminding him to trust her. She tried to inquire about Betsey's injury that night, but Mel interrupted them. After the drinks, the four of them walked home, with Mel subtly bringing up her challenges as a mother, hoping Jess would confide in her. Sadly, that didn't happen. Instead, Jess continued to keep her intrusive thoughts a secret. Mel switched tactics and point-blank asked Jess if she was okay and what she might be hiding.
As they talked, Kit climbed a tree against Jess's warning and ended up breaking an arm. Suddenly, Frankie started shouting, "He is dead. Frankie is dead!"
In her interview, Mel spoke candidly about the struggles of raising a toddler. She admitted that there was a time when she would have done anything except hurt the child to stop her crying. Meanwhile, Ed and Charlotte met with the lawyer who warned Ed that if he chose to be represented alone, he would be ending his marriage. After the lawyer left, Ed confronted Charlotte about what she told the police, surprised when she explained that she told them Jess was struggling and they were arguing. Of course, she acted as if she didn't mean to cause Jess any more trouble.
Just then, Mel called Ed from Jess's phone to inform him of Kit's accident. With two of their kids at the hospital, doctors warned Jess that things would escalate with all three children showing indicators of abuse. She turned to Mel for comfort. Ed and Charlotte arrived soon after to see Kit. Mel advised Ed to check the search history of Jess's laptop. Meanwhile, Liz reflected on her alcohol dependency and later checked on Betsey, only to be informed by Sheila that the baby had suffered multiple seizures that day. When Mel excused herself, Charlotte grabbed the chance to convince Ed to call the lawyer, arguing that Jess was an enigma and who knew what she did or didn't do to the kids? Jess overheard their conversation and panicked.
Ed texted asking where she was since Kit was getting discharged. She texted back telling him not to wait for her. For a while, she sat thinking about the nightmare she had been living in. The episode ended with Jess sneaking into the ICU and kidnapping Betsey.
