Mr. & Mrs. Smith Episode 5 opens with Toby lounging lazily in his abode, only to be abruptly ambushed by John and Jane. They swiftly cover his face with a bag, executing a mission assigned by Hihi: locating and retrieving Toby Hellinger from Lake Como. Their next objective is to safely transport him to a secure location.

As they travel, John and Jane hold hands, their fingers intertwined as they discuss their plans to purchase a new home. Toby, meanwhile, feigns a heart attack, cleverly manipulating the pair to remove the hood. They finally arrive at the safe house, but it's a trap. Dark-clothed figures emerge, guns blazing. The trio scramble into their car and flee, but they're closely tailed by another vehicle.

The chase through the narrow Italian alleyways is fraught with danger, Toby's knee issues and breathing problems making their escape all the more difficult. They eventually reach the serene shores of the lake and manage to hitch a ride with a friendly family, Jane inventing a pregnancy to secure their passage.

John leads them to a house, revealing it as a surprise purchase for them both. Jane is initially outraged, feeling as if John has made a major decision without her consent. However, her anger quickly dissipates as she recognizes his romantic gesture. While updating Hihi on their progress, she's asked to speak privately. Hihi inquiries about Jane's happiness, and she responds positively, praising John as a wonderful partner. Hihi seems pleased and encourages her to continue moving forward.

Later, Jane and Toby engage in idle conversation, Toby noting that John doesn't seem to warm to him easily. When Toby complains of stomach discomfort, Jane offers him medication, but he's reluctant to take it. John sweetens the deal, promising him cigarettes if he complies. Over dinner, Toby attempts to lighten the mood with a joke about encountering a Hitler lookalike in Argentina. Jane chuckles, but John remains unamused.

The evening takes a turn when Toby discovers John's empty promise of cigarettes. The argument that erupts ends in Toby's tearful retreat to his room. Suddenly, Hihi contacts Jane with instructions to rendezvous with a plane in the middle of the lake. Jane asks how they'll know when it's time to leave, but Hihi assures her they'll be notified. The conversation shifts to the mission, and John and Jane confess that their working styles don't always align.

John returns from consoling Toby, revealing that he'd been holding out on the cigarettes the whole time. Their conversation turns to weddings and the sentimentality of vows exchanged in person. John prompts Jane to suggest their own vows, and they exchange heartfelt promises, with Toby pledging never to harm her. Their shared moment of tenderness is interrupted by Toby's return, announcing that he's been sick. John rushes to his aid, leaving Jane alone with her thoughts and the promise of a new chapter ahead.

In the early morning hours, their adversaries tracked down the secluded house, but the trio managed to sneak away before the ambush could take place. They scurried to the lakeside, where a boat awaited them. Suddenly, a deafening explosion rocked the house, giving them a precious window of escape.

On the boat, John turned to Jane with a puzzled expression and asked how she had managed to cause such a commotion. She replied calmly, revealing that she had planted a dynamite stick on the stove. Just as Hihi had promised, a plane materialized on the lake's surface and swooped down to pick them up.

Upon returning home, Jane confessed that she wasn't feeling her best. John's hopeful gaze turned to concern as he asked if she might be pregnant. She shook her head, reassuring him that it wasn't that. He had misinterpreted the signs. John suggested that starting a family might not be such a bad idea, but Jane demurred. She didn't believe that their current lifestyle was conducive to raising children. John countered that he could find lower-risk work while still providing for a family, but Jane was adamant about continuing her ascent and embracing even higher stakes.

Just then, a knock on the door interrupted their conversation. A package arrived, containing a bottle of wine from Hihi. Attached was a note reading, "Well done, Jane." John's mood darkened, and he accused Jane of always taking the credit. He retreated to take a shower, leaving Jane to ponder the note. She messaged Hihi, asking if she was expected to have a child. Hihi's response was enigmatic, leaving the decision to Jane but hinting that she was ready to take the next step in their operation. Finally, Hihi posed a question that left Jane reeling: whether she was ready to replace John at the helm of their dangerous game.
