The sixth episode of My Royal Nemesis begins five hours before the flight, where Mo Tae-hee, from the Mochang Group, believes in efficient partnership for profit, much like Se-gye. He seems to be taking an interest in her. As the flight commences, he narrates that he hates meddlesome individuals, and he wakes up next to an electrocuted Kang. Given Kang's troublesome nature, Gwang-nam hopes to become Ji-hyo's manager. Coincidentally, Ji-hyo heads to Jeju to nurse her heartbreak.

In the hospital, Se-gye is worried about Kang. Son wonders why, as he lists all the misfortunes that have occurred since Se-gye met Kang. Se-gye finds Kang looking for him and they tussle over the remote of a huge TV in his VIP room, ultimately falling on top of each other. They freak out when Son spots them.
As for the poisoned meds and all the other scandals, Se-gye believes it's Mun-do's doing. In fact, Mun-do has blocked all articles about Se-gye's "health scare" to prevent their stocks from dropping. He only poisoned him to show how serious he is about replacing Se-gye.
Elsewhere, Tae-hee wins over Dal-su and his two daughters as she quickly updates them about Se-gye's welfare. During the car ride to the resort, Kang misunderstands Son's lactose intolerance and panics that there is a plague. Se-gye teases her and asks if it's because she rejected him. It's now his turn to panic as he tries to explain her words to Son. Kang also wonders if Mun-do poisoned Se-gye, even though he hides it from her.

Meanwhile, Mun-do puts a hit on An Seong-hui, the bribed nurse. Se-gye sends someone to investigate the poisoned meds and rushes her to the hospital. Back in Jeju, Se-gye and Kang are enjoying a pleasant time at the beach. It's her first time seeing it, and she's happy to be alive. He feels protective of her, but Son doesn't seem as thrilled about their closeness.
Ji-hyo is at the same resort as the rest and spots the group. Kang and Gwan-nam bicker over food, and Ji-hyo assumes Gwang-nam is the rich boyfriend. When Gwang-nam runs into her, he introduces himself as a fan, confusing Ji-hyo.
For the commercial, the outfits are revealing. Kang takes it in stride, adhering to the "if you got it, flaunt it" motto. However, Se-gye throws a fit, saying no to every outfit of hers. An amused Mun-do hears about it and says it's time.
After the wrap-up, an old man approaches Kang and asks her to look for his grandson in the woods, offering her candy. Se-gye learns about the hospitalized nurse and worries that Mun-do must have also gotten to Kang.
In the woods, the old man actually has dementia and thinks his adult grandson, Hyeon-ho, who works at the resort, is a kid who has been lost. Hyeon-ho comes to take the old man back and leaves Kang alone. At the same time, Grandma Nam worries that Seo-ri should have been back from school already. Does she have dementia too?
Back to Kang, her phone dies and she gets lost. The darkness triggers her PTSD from the time she was locked in a box. With Hyeon-ho's directions, Se-gye finds her and hugs her. However, Se-gye's fear gets the better of him, and he scolds her for meddling and not being able to separate private and professional life. She's hurt and distances herself from him.
Meanwhile, Mun-do meets with Tae-hee. Kang gets drunk to forget her worries. She orders Se-gye to take her to the mountains so she can start afresh as Shin Seo-ri. As a compromise, he takes her to the beach. She sees that he feels guilty for shouting and accepts his apology. He isn't sorry for worrying about her but sorry that he feels so much for her. He decides to kiss her and warns her about it. She knows she needs to stay away but can't help it. They kiss.
In the epilogue, we see that King has initiated a liquor ban. Prince Cheongheon finds Kang drinking and they joke with each other.
