Episode 1 of The Boys Season 5 begins a year after the events of Season 4. The episode opens with Annie hacking into a Vought computer while the annual shareholder meeting is in full swing. As Homelander gives his speech, we learn that Starlighters have been labeled a terrorist organization. Those caught are thrown into internment camps called Freedom Camps.

Annie projects a video onto the stage, showing Flight 47, where Homelander had threatened to kill the passengers in Season 1. Embarrassed, Homelander is about to kill everyone in the room when Sage stops him. She goes on TikTok and uses Firecracker's talk show, Deep's meninist podcast, and even Ashley, now USA's Vice President and a super who can read minds, to "debunk" the Flight 47 video and claim it's AI.

Homelander remains upset and demands everyone's devotion to him. The staff, Deep, Noir, and Firecracker are scared of him, which only annoys him more. He even berates those who like social media posts supporting Annie or share humiliating memes about him. The internet is also questioning the story of Ryan being at a boarding school, but Sage insists everything is exactly as it should be.

Homelander doubts her, especially since she's been stabbing her brain to get over her breakup with Godolkin. He disregards her plans and makes his own - he wants intel to be leaked that MM, Hughie, and Frenchie are to be executed in 3 days. This will lure out Annie and Butcher, whom he plans to kill. He also has Soldier Boy's cryotank in his room.

The scene cuts to Hughie, Frenchie, and MM in one of the Freedom Camps. MM has become a hopeless drunk. Frenchie befriends other inmates to get contraband and news on Annie. Hughie has lost all patience and tries to recruit people for a prison break. Most inmates have given up as the guards include supes like Love Sausage and Cindy. One of these inmates is Ivy, who worked with Hughie before.

As for Butcher, he visits his abusive dad who is still alive despite the cancer. Instead, his mom is dead. He reminisces about the times Sam Butcher made their family's life hell and declares that he's going to kill him now. Sam begs for mercy, but it seems that Butcher's mind is made up.

Next, Butcher pays a visit to Kimiko, who has been deported to Manila. She is wary of him, but he calls a truce to save the camp trio. His next recruit is Annie, who deeply resents him but agrees to help. She is thrilled to hear Kimiko speak, who now primarily converses in Gen Z slang thanks to the millions of TikToks she's watched.

The plan is to first rescue Frenchie, who is working on the super-killing virus—the only way to defeat Homelander. They know the mission is a trap, but they have no other choice. At the same time, the camp trio learns of the trap and plans their escape before Annie arrives to save them.

Ashley has a fake husband since no one will accept a single female super as their VP. Sage updates her that Homelander now wants to arrest all trolls. Ashley lists every organization, individual, and event that has been controlled, purged, cancelled, or imprisoned and wonders when it will end. Sage hints that if Ashley wants to be USA's President, she needs to play along.

Back to our truce trio, Annie contacts A-Train (Reggie) to help transport the boys to a safe location. He refuses, worried about his family. She understands and lets him go. Plan B involves flying each of them out one by one, which is more tedious.

At the camp, Homelander ambushes Hughie. He knows all about their prison break and has been toying with them. However, he wonders why Butcher and Annie are so devoted to someone mediocre like him to walk into a trap. Hughie claims it's because he would do the same for them.

The scene shifts to France where Reggie is hiding his family. He feels guilty that they are in this predicament because of him. But his brother Nathan insists he's proud of him for finally doing something good, like keeping his family safe.

Right then, the Deep shows up, threatened by Homelander for not capturing A-Train. However, Reggie is faster and quickly moves his family. Deep declares that he will never stop hunting him, but Reggie is not threatened. Instead, he observes that Deep is scared of Homelander and tries to indirectly turn him against Vought. Deep refuses but is clearly troubled.

As our truce trio makes its way to the camp, Butcher approaches Annie. She knew where the boys were held but didn't free them, citing that they are safer there. However, Butcher knows that she doesn't want to face Hughie for all the dark things she's done in the last year to take down Vought. He warns her not to let her guilt get them killed.

Back to the plan, Butcher blackmails Worm into digging a tunnel under the camp. He's a former Vought Studio writer who was fired and replaced by AI and also a super. Once he's done, he runs off. Kimiko and Butcher head in to find Homelander holding MM, Hughie, and Frenchie hostage. They keep Homelander distracted while he again monologues about how impressed he is by Butcher's devotion—that he ruined his own body to fight him.

Right on cue, Annie unleashes her starry power, momentarily blinding everyone with a dazzling display of light. In the ensuing chaos, the boys and the other inmates hastily make a break for it. Frenchie and Kimiko share a tender kiss as she whispers his name. Meanwhile, MM terminates Love Sausage's life. Annie is momentarily disoriented when Hughie abruptly ends their embrace to save Ivy from harm. As a guard attempts to shoot Frenchie, Annie swiftly whisks him and Kimiko to safety.

Homelander interrupts Butcher's skirmish with Cindy, causing her to flee the scene. Butcher hides, and to lure him out, Homelander ruthlessly kills Ivy and chases after Hughie. Butcher is too far away, and Homelander's laser beams are almost within reach of Hughie. Just then, Reggie appears, saving them all from certain doom. Homelander turns his attention towards Reggie, allowing everyone else to escape unharmed.

Hughie is visibly upset that Annie didn't stay behind to save everyone. Homelander is incredibly swift, and the two engage in a neck-to-neck race. Reggie almost makes it away unscathed, but a woman suddenly enters his path. He swerves to avoid her, leading to a crash. Homelander catches up with him, but Reggie is no longer afraid. He laughs at the irony of Homelander's helplessness without his powers. He laughs until the bitter end, until Homelander mercilessly breaks his neck.
