Shortly thereafter, Thony (Elodie Yung) and Jorge Sanchez (Santiago Cabrera) arrive at the chaotic scene. Jorge, with a steely resolve, commands Thony to keep Rex alive, to which she retorts that she is not endowed with miraculous powers. Undeterred, Jorge makes arrangements for a medical suite. Later, beneath the flickering surgical lights, Thony meticulously tends to Rex's wounds. Jorge enters, inquiring about his condition. She informs him of Rex's fractured sternum, collapsed lungs, and a potential brain injury, yet asserts that he will regain consciousness once the sedatives lose their hold. Jorge urges her to rest, but she insists on the critical necessity of monitoring Rex. Jorge reminds her that Rex was the getaway driver in their assassination attempt, yet she pleads for leniency, urging him to allow Rex time to heal.

Elsewhere, Jaz (Faith Bryant) politely asks Chris (Sean Lew) to tone down his music, only for Fiona (Martha Millan) to tease Chris by encouraging him to turn it up, attributing his smile to Ted's influence. The trio is unable to pick up Luca (Khalen Roman Sanchez) from school, prompting Thony to volunteer Stephanie's help. As Jaz departs with Luca, Thony expresses concern about placing too much on Stephanie's shoulders, which Fiona dismisses with assurance.

Meanwhile, Jorge expresses gratitude to Neto (Yancey Arias) for his arrival but cautions him against indulging in excesses akin to their Santa Maria escapade. Neto, remembering Jorge's aversion to boar hunting, places a cash roll bound with a broccoli band, adorned with rings, and a gun on the table. In the surgical suite, Jorge pretends concern for Rex, probing him for pain, only to signal Neto to continue his interrogation. Rex's screams of agony fill the room as Jorge turns a blind eye.

At the hospital, Dr. Dupont (Daniel Bonjour) grapples with an unruly patient, Ernest (James Cady). Thony deftly distracts Ernest while administering his medication, promising him a reunion with Edna (Sharon Skinner). Just as she receives a page, a scene shifts to the women's prison where Ramona Sanchez (Kate del Castillo) punches a heavy bag with ferocity. Chiqui (Patricia De Leon) kicks her, sending Ramona sprawling to the ground. Ramona rises, but Chiqui confronts her, accusing her of falsehoods about being a bruja. Ramona ignores her, but Chiqui persists, vowing to uncover her true identity.

Back in the surgical suite, Neto informs Jorge that Ramona's hired man, Feng (Alain Uy), orchestrated the hit. Jorge refuses to believe it. Thony bursts in, horrified by Rex's battered state, and scolds the men. Feng insists that all they need is for Rex to talk. Thony warns that if Rex perishes, he will take his secrets to the grave and demands Feng's departure. Feng taunts Jorge with a question, earning a stern warning. Thony frantically battles to save Rex, her hands flying with urgency.

Elsewhere, Fiona persuades Larry to extend credit for floor wax at her cleaning business. Krista (Diana Gaitiria) interrupts, informing Fiona of her plans to drive for StripRide using a fake American ID and boasts of her recent earnings. Fiona asks if Krista can help her secure a job.

In the surgical suite, Thony rummages through Rex's wallet. He confesses to driving the getaway car but denies knowledge of the employer's intentions. She interrogates him about the hirer, but he remains silent, pleading for her help. Reluctantly, she puts him into an induced coma and insists that Neto wait for him to awaken. Thony chastises Jorge for allowing Neto's brutality and insists Jorge is better than that. Jorge admits his doubts. As she exits, she pleads with Jorge to allow her to speak with Rex if he regains consciousness.

At Acapulco Tacos & Burritos, Fiona engages in a conversation with Elwood, portrayed by Michael Vincent Berry, regarding a StripRide gig. He reveals his scheme of renting out rideshare accounts for a weekly fee and hands her a forged ID belonging to Blanche Clutterbuck. Suspicion clouding her mind, Fiona makes a move to exit the conversation abruptly. Elwood suggests they start anew, and, albeit reluctantly, she agrees.

Within the prison walls, Jorge inquiries Ramona about Feng's loyalty. She responds by highlighting Feng's lack of familial support. Jorge vows to eliminate him discreetly, pending Rex's confirmation of Feng's guilt. Ramona insists he should make an example of Feng in public. As Jorge prepares to finalize a mining deal, he accuses Ramona of hiding in the shadows, recalling her days as Sin Cara. She advises him to convene a meeting, reminding everyone of the consequences for stepping out of line.

Elsewhere, Fiona fills Thony in on her StripRide job and the fake ID. Thony cautions her that she risks arrest, to which Fiona reminds her of her role as the cartel's doctor just as her first ride request pops up.

In Ted's car, driven by Chase Del Rosario, Chris performs donuts in a parking lot, attracting attention. After stopping, he tries to restart the engine but to no avail. Ted praises his driving skills, prompting Chris to inquire about Ted's plans for Cornell. Ted reveals it's his mother's idea, not his own. Meanwhile, Fiona drops Jaz off at a college fair, expressing her pride before departing. Shortly after, Fiona arrives to pick up her first customers, only for them to decline a ride upon seeing her van.

At the hospital, Thony is lost in thoughts of Neto torturing Rex, causing her to miss a Code Blue announcement. A staff member jolts her back to reality. She rushes to Ernest's room and initiates CPR, intubating him and successfully reviving him. However, DuPont threatens to report her to the medical board due to Ernest's DNR status. Thony apologizes profusely.

In a pub, El Don, portrayed by Danny Pardo, receives a call from Ramona, inquiring about Chiqui's father's revelations. He informs her that Chiqui's mother, Anita, passed out while smoking, leading to a house fire that severely burned Chiqui. Ramona orders El Don to spread the word that she fully backs Jorge.

Outside Belen Furniture, Fiona catches Chris and Ted driving recklessly. She reprimands Chris for neglecting his GED studies, and Ted, surprised by her still owning the red van, inadvertently offends her. Quick to defend her, Ted clarifies that his remark was harmless, but Chris pulls her away to smooth things over.

Jorge informs Thony that he needs Rex to talk before an upcoming cartel meeting. Thony administers adrenaline, reviving Rex momentarily. She reveals his internal bleeding and Rex pleads with her to give his wife a photo containing GPS coordinates of a hidden stash of money. She promises to ensure his family receives it and administers a medication to cease his heart's function. She informs Neto and Jorge, with Neto accusing her of murdering Rex and threatening her with a torch. Jorge orders Neto to dispose of the body.

Alone with Jorge, Thony admits to making Rex's passing painless but clarifies that Neto was the one who delivered the fatal blow. She shares that Rex denied any knowledge of the hit on them, hands over the GPS coordinates, and reiterates her promise to his family. She implores Jorge to let her assist him, but he instructs her to go home.

Back at her place, Fiona recounts to Chris how passengers refused her van. He comforts her with an embrace. Jaz arrives, upset that Fiona didn't pick her up, and accuses her of favoring Chris, who she believes doesn't pull his weight.

Jorge unearths Rex's bag, opens its contents with purpose, and declares confidently that he knows the identity of Rex's would-be assassin. He and Thony then make their way back to their car. At the prison, Ramona confides in Chiqui, sharing that her mother, Anita, has never forgiven herself for the severe burns she endured during the house fire. Reluctantly, Chiqui consents to Ramona reaching out to her mother. During their meeting, Jorge reassures the cartel members that all is well, presenting them with a lithium rock as proof.

Meanwhile, at the prison, Ramona places The Tower tarot card on the table, forewarning Chiqui that her carefully constructed empire is on the brink of collapse. At the hospital, Ernest writhes against his breathing tube, his distress evident. At the mining site, Jorge elucidates the significance of lithium to the gathered cartel members.

Back at the prison, Ramona startles Chiqui by revealing The Death card, urging her to confront her impending fate head-on. At the hospital, Thony discovers Ernest in a state of extreme agitation. Over at the mining site, Jorge strolls among the cartel members, dropping a bombshell: one of them is a traitor. Leveraging the lithium rock, he strikes Neto on the head, causing him to crumple to the ground. Jorge then throws a cash roll, secured with a quirky broccoli band, at Neto. Chiqui's stress levels escalate, beads of sweat glistening on her brow.

DuPont informs Thony that Ernest is fighting against the ventilator, momentarily distracting her. Neto draws a knife on Jorge, prompting a bystander to intervene, but Jorge insists on handling the situation himself. A fierce struggle ensues, culminating in Jorge plunging a knife into Neto's chest. Neto mocks Jorge, reminding him of his youthful fears. As DuPont and Thony remove Ernest's endotracheal tube, allowing him to breathe unaided, Chiqui collapses in a heap.

At the mining site, Jorge brutally beats Neto to death with the lithium rock, reassuring the cartel that he, indeed, is Sin Cara. Ernest miraculously resumes breathing on his own. While treating Jorge's arm laceration, Thony inquires about the meeting's proceedings. He entrusts her with a bag of money to deliver to Rex's family. Elsewhere, Chris and Ted delight Fiona with her souped-up van, and she eagerly takes Jaz for a spin.

Edna informs Thony that she bid Ernest farewell before his passing, thanking her with tears streaming down her face. Thony offers her comfort. Drinking heavily, DuPont proposes a toast to Thony, acknowledging that he has officially earned his hatred. Sipping from his flask, he ponders whether hell is indeed other people or perhaps the all-consuming loneliness.

Later, Thony drops the bag of cash at Rex's residence, ringing the doorbell and observing as Rex's wife, Sara Starr, peers inside the bag before carrying it indoors. Unbeknownst to her, someone snaps a photo of her as she returns to her car.
