At the hospital, Shaun gathers a cadre of doctors to brainstorm potential treatments for a purportedly mysterious patient. However, Lim, played by Christina Chang, swiftly uncovers Shaun's ruse and withdraws her support, realizing that the patient in question is none other than Glassman. Instead, Lim assigns Jordan and Kalu to focus on Claire's case, leaving the newlyweds Park and Morgan to join Shaun's team alongside Dom and Charlie. Park urges everyone to shelve their ideas until Shaun accepts the reality of the situation.

The cause of Claire's collapse is quickly identified as a bacterial infection known as "Acinetobacter," a resilient strain resistant to antibiotics. Despite attempts to debride the infection, it remains unchecked, and Claire succumbs to septic shock. Realizing the gravity of her condition, Claire prepares herself for a medically induced coma. Lim and Kalu exchange emotional farewells with their beloved colleague and friend.

With a sense of resignation, he doubled down on the necessity of amputating her arm. Shaun, unable to hide his dismay, pointed out that such a move would effectively strip Claire of her identity as a surgeon, her lifelong passion and calling.

Kalu, however, countered with a profound observation. He argued that while being a surgeon was indeed a significant part of Claire's life, her true joy stemmed from helping others, a calling that would remain intact even without her arm. With a heavy heart, Lim concurred, and the decision was made to amputate Claire's arm.

The atmosphere was filled with an unspeakable sadness as Lim, Morgan, and Lea shared tender farewell scenes with Glassman. Shaun, however, seemed to be in a state of denial, unable to accept the reality of the situation.

Just when it seemed like all hope was lost, Park managed to reach Shaun with a potential breakthrough. Coupled with some experimental ultrasound therapy, this could potentially give Glassman up to a year to live.

Lea, seeing the toll this was taking on both her husband and their friend, gently confronted Glassman, imploring him to be patient with Shaun. During this emotional exchange, Shaun finally accepted Glassman's condition, realizing that while he could do his best to help, ultimately, it was Glassman's life to live as he chose.As Lim delivered his criticism, accusing Shaun of being an unsupportive friend, Shaun experienced a moment of epiphany. He realized that he could potentially save Claire's life by harnessing the power of viruses to combat her infection, specifically by utilizing bacteriophages. However, the task of locating the precise bacteria required for this treatment was immense and quite unsanitary.

Shaun paused by Claire's hospital bed, overwhelmed with guilt for not being there for her earlier when she begged to see him. Tears streamed down his face as he gently clasped her hand and bid her a heartfelt farewell. Suddenly, Charlie roused Shaun from his sorrowful reverie, announcing that they had discovered the crucial bacteriophage.

Filled with renewed hope, they embarked on a mission to secure compassionate use approval from the FDA. Alas, their efforts were met with rejection. Undeterred, Shaun resolved to forge ahead with the bacteriophage treatment despite the FDA's bureaucratic obstacles. He understood the gravity of his decision; disregarding the FDA's regulations would mean forfeiting his medical career.

Glassman, seeing the dilemma Shaun faced, intervened. He believed that Shaun shouldn't have to sacrifice his future for Claire's sake. So, Glassman stepped in to administer the bacteriophage treatment himself, thus saving both Claire's life and Shaun's professional future.

A flash forward unveils a touching scene where Shaun fulfills his promise and visits Maddie's carousel, accompanied by Glassman. The two share tender words of "I love you(s)," and later, Shaun rides alone, symbolizing Glassman's passing into the next chapter. As time passes, an older Steve and pregnant Lea join him for a spin, and eventually, the trio expands to include Steve, Lea, and their adorable daughter.

Lim, despite her illustrious position and a lucrative offer to become the Chief of Surgery in Chicago under Clay, opts to travel to Ukraine to work with Surgeons for a Better World. On the domestic front, Park and Morgan joyfully play with their formally adopted toddler, Eden, their family life filled with warmth and love.

Dom has transformed into a skilled physician at Hubank Community Health, confidently tending to a bloody wound without hesitation. Jordan belts out tunes in the church choir, accompanied by a beaming Perez. Charlie, now a seasoned mentor, guides a new generation of residents through the hospital halls, the student having become the esteemed teacher.

During a captivating TED talk, Shaun reveals that he and the vibrant Claire have successfully helmed the Dr. Aaron Glassman Foundation for Neurodiversity in Medicine.
