June, however, remains grounded in realism, having navigated such perilous terrains before, only to find herself ensnared in miseries anew. Despite her arm throbbing incessantly from the train accident, exhaustion eventually claims her, and she drifts into a much-needed slumber. When she awakens, she discovers Nichole missing, only to learn that Serena, seeking to afford her some rest, had taken the child. June brushes it off with a forced joke about Serena stealing her baby, to which the latter responds with an equally awkward chuckle.
The train, now teeming with refugees after a six-hour layover in Winnipeg, grows increasingly crowded. June's condition worsens, prompting Serena to intervene and summon a doctor. The physician diagnoses an infected arm, explaining her feverish spells, and does his utmost to treat her. Post-examination, June catches up with Moira, who delivers the somber news of Luke's arrest. Though this is old information, Serena's words offer no solace; Hannah remains imprisoned in Gilead.
To compound their woes, it transpires that this train is not destined for Vancouver. Overflowing with passengers, it refuses to take on more. Their ultimate destination remains a mystery. What they do know, imparted by Mark to Moira, is that relations in Toronto are fraying, with perhaps a year left before everything collapses into chaos.
Much of this situation arises from the Cabinet's desire to commence normalizing ties with Gilead, a move that entails derecognizing the United States in the process. It seems apparent that resistance, at this juncture, is a losing battle, despite Mayday's clandestine resolve to unseat this oppressive regime and thwart such an occurrence. As events unfold, Moira steps forward to lend her aid.
Throughout these tumultuous times, Serena vows to assist June, pledging to restore their family's unity after the harrowing ordeal they've endured. However, her incessant invocations of God's Grace and His plan fall flat among many of the women aboard these carriages.
Meanwhile, the astute physician from earlier has conducted his due diligence and recognized Serena's Gileadean origins. He underscores her perilous nature and suspects her travel under false pretenses. Indeed, an officer is en route to apprehend her. It transpires that this doctor once served as a fertility specialist in Gilead. When he utters Serena's name, loud enough for the other women to hear, June finds herself in a precarious predicament.
A moment later, an officer arrives, presumably from another carriage nearby. He verifies the authenticity of her papers and leaves her among the other women, their thirst for vengeance palpable. He does so deliberately, fully aware of Serena's identity and the violent consequences that could ensue.
Desperate to save Serena, June pulled the emergency brake. She convinced Serena to escape the train. Consequently, June is branded a traitor as Serena slips beyond capture, while she herself reaches her intended destination – Alaska.
In the town of New Bethlehem, Nick arrives at his new abode accompanied by Rosie, where he engages in a conversation with his new father-in-law, Commander Wharton. The encounter serves as a stark reminder that his freedom is but a distant dream. He is gently yet firmly reminded to let go of his "juvenile fantasies" — a euphemism for his yearnings for June — and embrace his new beginnings with open arms.
Switching gears to a different chapter in life, June and Nichole are taken aback to find themselves unexpectedly reunited with none other than June's mother, who coincidentally holds a position as a physician in this very place.
