Episode 7 of "The Pitt" seamlessly continues from the cliffhanger, with Kristi still confined within the bathroom's four walls. Robby, unable to contain his frustration over the ER's overwhelmed state, vents his pent-up emotions to Gloria, painting a picture of endless queues and the dire consequences they breed, such as Kristi's predicament. Gloria, sensing the tension, excuses herself discreetly.

Dana, ever the playful one, ribs Robby about his hunger-induced irritability, but beneath the banter lies a harsh reality: it's been an arduous day marked by an unyielding influx of patients. Compounding the chaos, Rita remains mysteriously absent, and Nick lies in a vegetative state. Adding to the mix is a new arrival, Terrence, an autistic individual with a sprained ankle who isn't particularly receptive to Dr. Langdon's bedside demeanor. Finding his approach unfruitful, Langdon steps aside.

Enter Melissa, who quickly wins Terrence over with her gentle rapport. Her patient interaction skills leave Langdon thoroughly impressed, prompting him to offer heartfelt praise. Melissa, touched by the commendation, shares that her proficiency stems from her close bond with her sister.

Elsewhere, Santos ventures into the bustling waiting area to break the news to Silas's mother, Susan, that her husband has developed breast enlargement, possibly due to hormonal imbalances or, ominously, alcohol abuse. Tests will clarify further, but we soon learn the truth without awaiting their results.It transpires that Susan has been surreptitiously lacerating Silas's coffee with progesterone each dawn, her malevolent aim to quell his sexual desires. She harbors a deluded conviction that Silas is preying upon their daughter. However, Santos's thoughts are abruptly derailed when a new patient, Nandi, storms in accompanied by her roommate. Nandi, exhibiting signs of psychosis, has been hurling curses at passing traffic and losing her composure all morning. She scorns everything around her, sparking myriad hypotheses, ranging from schizophrenia to substance abuse. The medical team is now engrossed in unraveling the enigma surrounding her condition.

Concurrently, Dr. Collins engages in a heart-to-heart conversation with Kristi's mother in the waiting area. The latter is stone-cold and insists that her daughter has had ample time to come to terms with her pregnancy. Eloise, drawing from her own experience of becoming pregnant at Kristi's age, although arduous, asserts that her journey with Kristi has been overwhelmingly positive—until now, of course. Dr. Collins, speaking candidly, cautions her that the more she pressures Kristi, the greater the risk of alienating her daughter entirely.

Meanwhile, we revisit the saga involving Minu, where we gather incontrovertible evidence that she was forcibly propelled onto the subway tracks. Sam, emerging from a six-episode coma, vows to make a formal statement to the police. Santos confronts Robby with her qualms about Silas, reiterating her belief that Susan might be right, urging them to take decisive action. However, the dilemma persists; they lack concrete proof, and the solitary fact they can ascertain is that Silas was drugged, which led to his fall from the ladder. Unless the daughter comes forward, their hands are tied.

Naturally, Santos turns to Alana, the daughter, and without hesitation, brings up the topic of molestation. The conversation quickly becomes tense and uncomfortable, forcing Alana to seek refuge with her mother. Meanwhile, we discover that Nandi is a prominent beauty influencer online, and her symptoms could potentially be linked to the products she imports from overseas. These products, it turns out, contain dangerously high levels of mercury.

Robby is incensed that he wasn't consulted sooner about the situation, still clinging to the notion that they would refer the matter to the psychology department. He confronts Dr. Mohan for failing to adhere to the proper chain of command. When Robby discusses this with Dr. Collins, she mentions Adamson and reveals that Mohan's confidence is severely shaken. They are all aware of his fragile state and his tendency to snap at everyone.

Robby remains on edge and seeks out Susan, informing her that they must involve the police. Santos reacts poorly to this news and retreats to speak with Silas in private, calling him a despicable person. It transpires that Santos has personal experience with such matters, explaining her intense reaction. She makes her feelings known without any reservation.

As the episode draws to a close, Kristi and her mother reach a consensus and depart. Meanwhile, Dr. Collins begins to feel a painful discomfort in her stomach. She heads to the bathroom and discovers that she is bleeding heavily.
