Langdon provides an update to Robby on the medications, and they delve deeper into the condition. Initially, they speculate it could be SIRS, but Debbie's boss interrupts, ringing with a threat to fire her if she doesn't show up soon. Robby steps in, taking the phone and shielding his patient from dismissal by threatening legal action if the boss dismisses her.

With this hurdle cleared, Langdon and Robby clash over the potential diagnosis. As Debbie's condition deteriorates, they realize it could be sepsis, requiring immediate surgery. The surgeons eventually descend to assess her, initially requesting more scans, but Robby refuses to wait, insisting they operate immediately. Thankfully, the surgeons take Debbie for the treatment she desperately needs.
In the midst of this chaos, Robby dismisses Langdon and takes him out of the ER. There's still hesitation over Robby's ordering of medications, and Whitaker doesn't help by rubbing him up the wrong way while examining Louie. Whitaker jumps in as the acting physician and orders medications for him.
Meanwhile, there's no update on baby Doe. They continue searching through CCTV cameras for answers, and I'm sure we'll get some clarity in upcoming episodes. In the meantime, Jackson's sister heads to the cafeteria while her brother remains unconscious. Baran and Javadi suggest she pursue accountability, but they need to wait for results to come back before knowing anything definitive.
Another patient from last week, Orlando, embodies the struggles of poverty-stricken Americans. He refuses further treatment due to costs and continues to calculate sums in his head. Mohan eventually manages to negotiate with him and his wife to keep the insulin drip, but the hospital can't cover all costs. Joy's ingenuity strikes a deal, and Mohan commends her for her resourcefulness, explaining that they used to have an uninsured grandma and had to work around the system.
As all this chaos unfolds, the Emergency Room is rapidly filling up. Along with Roxie, a patient suffering from seizures, another man, Gus, a victim of a brutal assault, is wheeled in. However, he's an inmate and needs to remain cuffed for the time being. Gus is in a terrible condition, with multiple rib fractures and a fractured mandible. He appears to have fragile skin and is in a state of severe distress. Unfortunately, more patients are being brought in from Westbridge, and Robby is concerned given that the entire facility has gone radio silent. Dana has struggled to reach the charge nurse there, who is unresponsive.
Meanwhile, Santos continues her charting work, fobbing off every single patient to either Ogilvie or Whitaker. When Baran suggests using generative AI to speed up the process, Robby pipes up, worried that the hospital would just take advantage of them if they did this.
As the episode comes to a close, Louie crashes through, and both Robby and Langdon are on the case.
