She takes out Anna's security detail without a thought, and Dex's desperate attempt to assist in Anna's imminent delivery ends in tragedy when whatever is being born severs his hand.

Flashbacks reveal Siobhan's calculated move with Dr. Hill—a game of cards where she promises him wealth and fame in exchange for unquestioning obedience. But there's a catch: She makes her own rules.

Back in the present, the ambulance comes to an abrupt halt in a dimly lit garage. Ivy's brutal act of shoving Dex's hand into his mouth sends shockwaves through the already fraught atmosphere. Anna, now on a stretcher, is confronted by a group of black-clad women who urge her to push. As the baby is born, they declare its perfection but deny her the right to hold her own child. Ivy reveals the heart-wrenching truth—the Oscar was the price she paid for Anna's newborn. Enraged, Anna rises to reclaim her baby only to be stabbed in the back. They implore her to accept her fate or face a similar end to Mrs. Preecher's.

Siobhan, revealing herself as Dex's biological mother and his father's lover, adds fuel to the fire by accusing Anna's paralysis on herself for trying to take back her child. She presents Anna with a choice: join their ranks and be rewarded with youth and beauty or suffer the fate of those who resist—like Mrs. Preecher. The twist of Anna giving birth to the most potent vessel of all is shocking, and Siobhan hints at a sinister bond between them—a bond that would see her embrace their cause if she held her grandchild.

Anna is transported to a sinister nursery, where Siobhan avenges Dex's death by taking Ivy's life. Siobhan reveals her macabre plan to preserve Dex's reproductive organs, a testament to her unwavering dedication to creating a new race of beings, one that includes Anna, in a bid to save humanity. Her ultimate goal is to establish a matriarchal society—a utopia by any means necessary. Siobhan, the Auteur who rules the world with an iron fist, hands Anna the infant, whose tiny fists are already adorned with claws. As Anna feeds him, the child bites and sips her blood, the pain serving as a grim reminder of the motherly bond that Siobhan celebrates as both excruciating and exquisite.

Adeline, alone and clad in white, sings to the infant and weeps, her prayers echoing those from her restaurant. Anna joins in, and together their chants unleash a force that shatters the mirror and consumes the creature-child. The only remnant is Siobhan's headpiece, which Anna dons, symbolizing her transformation.

The episode concludes with Anna hearing the cry of a baby and discovering an all-white nursery containing a normal infant in a crib. Before picking up the child, she brushes aside a spider and kills it. The sound of whispers reaches her ears, and she realizes she holds her Oscar and her newborn.
