In the year 1967, New York City is the setting for American Horror Story: Delicate's eighth episode, where we find Frank Sinatra and Mia Farrow sharing a restaurant table. Sinatra lays down an ultimatum to Farrow—choose between him or your career, as the love between them hangs in the balance. Farrow, pregnant with her Rosemary's Baby, relents to a director's demand to walk into traffic for a gruesome scene, unaware that no one would actually hit a pregnant woman. But as she reluctantly complies, her dressing room becomes a scene of chaos; she bleeds profusely, and her fake belly begins to move in an unsettling manner. Siobhan, an enigmatic figure, steps in to save the day, urging Farrow to remove her belly and promising to make everything stop.

Fast forward to the present day, and Dex stands at his mother's funeral, delivering a poignant eulogy. Mrs. Preecher interrupts, her voice laden with urgency, to reveal that his mother did not take her own life and had tried to warn Dex many times before. As Anna leaves the scene, Dex's father can't help but make snide remarks about her size.
Anna discovers that Mrs. Preecher has been taken to the ER for sedation and finds her there, begging Anna to leave now before "they" take everything from her. As Anna waits with her, Mrs. Preecher is whisked away by "friends." Meanwhile, Anna learns of her Oscar nomination, signaling a shift in her career trajectory.
Back at their work space, Anna and Siobhan are focused on their outfits for upcoming events when a strange buzzing from a phone call disrupts their routine. It's not either of their phones; instead, they find a flip phone belonging to Dex in a drawer. The call comes from an unknown number, adding to the growing list of mysteries surrounding him.
In Los Angeles, Anna visits the gifting site where she encounters a mysterious dark hooded figure. As she watches someone remove their jacket, she realizes it's Cora from Dr. Hill's office. Cora reveals herself to be the one behind Anna's troubles—the one who messed with her appointments and deliberately tried to ruin her life. With each new revelation, the web of deceit and horror in American Horror Story: Delicate tightens its grip on its audience.
Cora, her voice tinged with urgency, confides in Anna that "they" are orchestrating a sinister plot against her unborn child, insisting that Dr. Hill is collaborating with individuals who engage in heinous practices against his patients. As the night wears on, Anna confronts Dex with a resolute command to leave. Awakening in the solitude of her bed, she discovers that her legs have transformed into unfamiliar, scaly apparitions. In the bathroom's hushed stillness, she delves into the strange sensation, realizing that it originates from within her very being.
The Oscars loom large, and Anna catches a fleeting glimpse of Babette on the red carpet. She stumbles momentarily, assuring Siobhan that her body would be the first to alert her of any malady, and the show must go on. Amidst the audience's hype, Anna believes she experiences the onset of contractions, but Siobhan urges her to hold tight until her category is called.
As Anna reveals to Siobhan her willingness to forsake all for the coveted Oscar, a sudden announcement interrupts her thoughts—she has been named the winner. As she prepares to deliver her acceptance speech, the crowd vanishes, leaving only the applause of her mother echoing through the void. This is followed by the telltale sign of amniotic fluid leakage. The episode ends with Anna snapping back to reality, her journey abruptly cut short as she is escorted from the stage.
