The fourth episode of CIA commences with a gripping scene in Belarus. Daniel, a CIA operative, is on the ground, his face battered and bruised. He's pleading with his captors, who are demanding to know about the money. "I don't know," he repeats weakly.

Suddenly, a sack is brought before him. As he opens it, his heart skips a beat. It's the head of his wife. He begs his captors not to kill his son. But it's too late. Aleks, one of their henchmen, shoots Daniel dead. Their mission has changed, and Aleks is now sent to America to complete the transaction. He picks up the pregnant young woman and leaves with her.

Meanwhile, Colin is taking a polygraph test. He confesses to lying and betraying someone. Elsewhere, Bill is video-chatting with his parents, who inform him that his father's cancer is in remission.

Back to Colin, he confesses to having been posted in Mindanao, Philippines. His partner, Toni, was killed in an explosion at the Burma Bar. A month later, he requested a transfer to the NY station, saying he liked working with Bill. However, his new partner, Bill, doesn't know that his partner died in the field.

Bill's mother informs him that they visited an oncologist and his father is doing well. Bill took the test and wants to do it for his father too. Later, Colin asks Nikki if she knew he was going to get swirled (i.e., interrogated under duress). She says no and tells him to stop looking for a mole and that Toni died for no good reason. Bill walks in, and they get briefed.

The office has received intel from Poland's ABW about a male from Belarus headed to JFK. It turns out to be Aleks, and he's from Minsk. He's a member of PATH, a Neo-Nazi accelerationist recruited from foster care. The group is hierarchical, and he has moved up the ranks. They need to know his mission so Colin, Bill, and Gina head to the airport. Gina spots Aleks at the airport with the young, pregnant woman and informs Colin, who is outside.

The young woman was gently assisted into an ambulance, with agents closely following their lead. In the hospital, Aleks made a call in Spanish, while Bill discreetly recorded their conversation. A nurse approached, informing Aleks about the child, but his focus seemed to be more on the infant than its mother. It was revealed that the lady's name was Sophia. Shortly thereafter, the nurse informed them that Aleks had run off with the child.

Meanwhile, Gina updated Nikki on Sophia's situation. Sophia had no criminal record, nor did she have any parents or siblings. The hospital had issued an AMBER alert for the premature baby, who had high bilirubin levels and required immediate medical attention.

In the hospital, Colin posed as a child services officer, seeking to gather information from Sophia. She revealed that Aleks had run away with the baby because she had changed her mind about selling the child. He had promised her $2,000, a higher price than the $1,000 women in her village typically received. Aleks had promised her that her baby would have a better life in America. Colin handed her $1,800 and promised to help her get her baby back. Sophia also revealed that Stephanie Harris was the woman who was meant to adopt the baby.

Bill and Colin traced Stephanie, who recognized Aleks as the owner of the adoption agency she had been working with. She had wired $100,000 for a baby boy who was due to be born in a week. She was supposed to wire another $50,000 once she received the baby. She claimed that she had tried to adopt, but those programs had been closed.

Bill cites abuses as the primary reason for the closure of most operations. They inform her that Sophia gave birth, but Aleks took off with the baby. She met Aleks on his adoption website, and he helped her make a video for the birth mother. Stephanie also mentions Liz, the adoption attorney, who was working with Aleks. Meanwhile, the agents head to Elizabeth's office but find her absent. Colin sneaks into her office and copies a file onto a flash disc. However, Bill states that they cannot use the copied file since Colin obtained it illegally.

Meanwhile, Gina decodes Aleks' message, which reads "the attacks will not be delayed." Bill and Colin head to court, where Elizabeth is having a civil court case. Bill confides that his father is sick and that he sacrificed a lot for him. They question Elizabeth, who reveals that Sophia was a surrogate for a couple who changed their minds. Together with Aleks, they agreed to find another baby for the couple. She brushes it off and asks them to leave.

Back in the office, the team realizes that Stephanie and other wealthy Americans are funding grants for having babies, which is how terrorists are funded. Nikki suggests following the money, but Gina believes they should follow the girl who might lead them to Aleks. She asks Gina to find every detail, including the driver who drove Aleks off.

Back at the hospital, the agents receive information about other women using the same address. Colin lures the nurse and gets the address, which she had earlier refused to give out. Outside, Bill's mother reveals to him that his father's health is deteriorating. He's just putting on a brave face. Colin takes over the phone for a fun chat with Bill's mom while Bill is lost in thought.

The next scene shifts to the given address, where agents find pregnant girls. It's a baby factory. They're informed that Sophia came looking for her baby, but they haven't seen Aleks either. One of the girls points out that Sophia is upstairs. However, they find out she committed suicide. The house is treated as a crime scene, and the girls give their statements.

Back in the office, Path takes credit for the bombing in Alabama, where five are confirmed dead. Nikki says they're planning more terror attacks in New York, though there are no details yet. They inform the team that Aleks strangled Sophia since the time of her death coincides with the time he was in the house.

Gina alerts them to the getaway car found abandoned. Meanwhile, Elizabeth is questioned, claiming that Aleks is not a Neo-Nazi but is raising funds for poor kids affected by the Russian war. However, they inform her that he lied and his group carried out an attack. They need to find Aleks and the driver now.

Another attack has taken place in a church, resulting in the deaths of three people. They lost track of Aleks and the driver. Stephanie is their next hope. Bill visits her, and she reveals that Aleks informed her of Sophia's death and said the baby is sick and needs money for treatment.

The team sets up a meeting between Stephanie and Aleks, who gives a street address. Once he grabs the money, he drops a bag but the baby isn't there. They chase after him and apprehend him inside a bus. Bill and Colin trace another baby factory house where they find the baby in critical condition.

The scene shifts to the hospital where Stephanie is holding the baby. Bill informs her that they spoke to social services and she fits the bill to be its mother. He also reveals that Sophia was an orphan. Back in the office, Nikki informs Bill that Aleks will be arraigned in court. They also found other locations where Path was hiding ammunition. She says today was a win and invites him for a drink.

The episode ends with the team drinking out where Colin encourages Bill that his father will be alright.
