Her memories flit between a tender kiss shared with Michael Hamda (Omar Metwally) and the harsh accusation hurled by Richard Miller (Scott Wolf) – that she was responsible for Bill Dixon's demise. Compelled by curiosity, Amy dials TJ's number, seeking an update despite her expulsion from the case. Katie (Charlotte Fountain-Jardim) enters, her concern evident as she checks in on her mother.

The harrowing news of a train derailment breaks, captivating Amy's attention. Nine lives lost, 43 bodies bruised and battered. Without hesitation, Amy rushes to the hospital, her heart pounding with the urge to aid the suffering. Meanwhile, Michael, ensuring the mass casualty incident protocol is in place, absently leaves an item in his vehicle. Nora (Sarah Allen) attempts to catch his eye, waving the forgotten object, while her tablet buzzes incessantly with messages from Amy, eager for a conversation devoid of last night's revelations.

Amy catches up with TJ, who delivers a somber pronouncement – the hope for reversing his father's paralysis has vanished into thin air. Amy grapples with disbelief, clinging to the shreds of her confidence. Richard addresses the team, the toll rising to 84 wounded souls, preparing them mentally for the harrowing scenes awaiting. In a blur, patients are wheeled in, one woman hailing a hero amidst the chaos – Bradley Hahn, who bravery saved countless lives from the blazing trainwreck. Modestly, Bradley dismisses his actions as any decent person's duty, while Jake Heller (Jon-Michael Ecker) struggles to find him some respite.

Richard confides in Amy, weaving tales of redemption through mistakes made and lessons learned, urging her to embrace the future. Yet, Amy's heart remains heavy, uncertain of her ability to move past the shadows. Elsewhere, Gina Walker (Amirah Vann), anxiety etched on her brow, desperately seeks information, enlisting Nurse Julie (Claire Armstrong) to ascertain if Ian Flores (Aristotle Domingo) has been admitted. Jake attempts to delve into Amy's conversation with Richard, but she rebuffs him, her mind elsewhere.

Amy steps away to lend a hand to Nurse Liz (Conni Miu) attending to Melody Foy (Melanie Leishman), whose wedding bells are now tinged with uncertainty due to her ordeal. Melody's ankle, a grim testament to the crash, demands immediate attention. Amy, noting Jeremy Hardy's (Al Mukadam) injured arm, directs Liz to care for it while probing Melody's pain. Summoning Sonya Maitra (Anya Banerjee) for assistance, Amy gently ushers Jeremy to the waiting area. Gina, phone glued to her ear, tirelessly scours other hospitals for Ian, her conviction unwavering – Ian is her patient, and she believes he orchestrated the disaster.

In the hectic backdrop, a heated debate ensues between TJ, Julie, and Sonya over a patient's potential survival. Jeremy, love-struck and desolate, seeks solace in Amy's words, only to crumble moments later. Amy springs into action, her voice commanding an airway cart, her focus unyielding.

Reverting to her initial disdainful demeanor, she alienates those around her. Sonya, puzzled by this sudden change, inquires into the cause of Amy's behavior. Amy unleashes a tirade, leaving Sonya distressed and ultimately exiting the scene. Jake, however, asserts that his son would undoubtedly be filled with pride today.

Theresa Coleman, portrayed by Cherion Drakes, exchanges words with Randy, played by Demore Barnes, while TJ observes them from the doorway, captivated by the interaction. Michael disregards a call from Nora, opting instead to inform Julie about the blood drive organized downstairs. He also updates Gina on the absence of Ian Flores from the latest casualty list, only to be interrupted by Amy's curiosity about their conversation. As Michael storms away in frustration, Amy seeks clarification on the ongoing situation. Together, they witness the police escort Ian, the driver, into the hospital. Richard's shouted reminder that Ian is a patient pulls everyone back into focus, urging them to fulfill their duties. Gina asserts her role as Ian's attending physician to MPD Sergeant Dove (Thomas Mitchell), who reveals that Ian's final call was to her. Despite her insistence of ignorance about his intentions, Sergeant Lee (Evan Annisette) suggests otherwise, implying she might be withholding information.

Amy and the team strive to save Ian, but the odds seem stacked against them. Meanwhile, Sonya lending her expertise to Ziggy (Tommy Tang), they cling to hope that prolonged CPR might revive him. Gina plays Ian's call for Michael and the MPD officers, revealing his torment over accidentally hitting an 11-year-old girl, despite witnesses clearing him of fault. Gina speculates that Ian's desire was not to derail the train but to end his own life, caught in a spiral of guilt. TJ and the rest of the team battle to save Ziggy, while Michael informs Gina of the police's departure. Sonya detects a pulse in Ziggy, who miraculously opens his eyes. Nora enters, catching Michael midst a work call, and inquires about the previous night's incident with Amy. Nora agrees to discuss it further at home but crosses paths with Amy on her way out, expressing her enduring love for Michael and blaming Amy for shattering him. Overhearing this exchange, Sonya catches snippets of Nora's harsh words to Amy, emphasizing that she had her chance and squandered it.

As Katie enters the room, Amy is seen leaning casually against the bathtub, her posture betraying a hint of vulnerability. Upon noticing Katie's arrival, Amy straightens up and dabs at her eyes with a tissue, an action that doesn't escape Katie's notice. Amy, however, mentions prescribing him a beta-blocker beforehand, to which Patty firmly denies, reminding her of Bill's hypotension.

Meanwhile, Michael stirs from his slumber on the sofa, the ringing of his phone jolting him awake. Nora's voice, tinged with reproach, scolds him for neglecting to return home. He attempts to clarify, but she abruptly terminates the call. Elsewhere, Jake shows his patient, Bradley, a news clip heralding his heroism in rescuing passengers from a train derailment. Joey Hahn (Declan Cooper) shares his pride in his father with WDZT reporter Mackenzie Cole (Melanie Ng). Moments later, Bradley's wife (Kerry Ann Doherty) and Joey arrive, and Jake watches as Bradley embraces Joey warmly.

TJ approaches Amy and envelops her in an unexpected hug. Amy, caught off guard, inquires about the reason behind his gesture. He acknowledges her correctness about his father's ability to walk and apologizes for his earlier behavior. Amy reassures him that nothing could alter her opinion of him. Grateful, TJ thanks her and moves on. Jake, sensing an unspoken tension, asks Amy if she's been avoiding him. Liz catches Amy's eye as she passes by, prompting Amy to promise Jake she'll share more later. Rushing to catch up with Liz, Amy demands the truth from her, her tone urgent and resolute.

Amy imparted to Michael and Max Garner, alias Darrin Baker, that Richard had persuaded her into believing she had prescribed Bill a beta blocker. During a subsequent meeting, Amy confronted Richard, citing information imparted by Patty, accusing him of administering Metoprolol and ultimately causing Bill's demise. Flashing back four months, Richard had received a call from Minneapolis Police Officer Fornay, alerting him to Lucas's arrest for vandalism and resisting arrest. Richard insisted that Lucas was mentally unstable and must be released, to which Fornay responded that Lucas would be detained in a holding cell until someone claimed him. Meanwhile, Bill was apprehended, and a medical team scrambled to revive him. Richard administered defibrillator paddles and ordered 300 milligrams of Amiodarone. Liz attempted to collect a syringe but dropped it, bending down to retrieve it. Seeing this, Richard stepped in, drew the medication into a syringe, and administered it through an IV line. However, Bill's condition deteriorated. Realizing his mistake in administering Metoprolol, Richard quickly drew Amiodarone into a syringe, accidentally dropping the vial to the floor.

Returning to the present, Richard confessed his actions. Garner warned him of impending disciplinary action and the suspension of his license. Richard apologized to Amy, but she could not forgive him for attempting to frame her during her most fragile moment. Garner reminded Amy that she had violated hospital policy by speaking to Patty. She countered that there would be no consequences for her actions and demanded that he issue a check to Patty. She declared her intention to cease her accommodating demeanor towards the board and hinted at potentially reconsidering the chief position in a few years.

Jeremy awoke to find Amy gazing at him. She informed him of a brain bleed but assured him they had caught it in time, emphasizing that his current condition was temporary. Sharing a personal anecdote, Amy revealed that she had experienced something similar a few months prior, urging him not to give up. Moments later, Melody was granted permission to see Jeremy, while Gina prepared to leave, citing the need to address personal matters. Amy inquired about her wellbeing, but Gina responded curtly, stating that yesterday was the appropriate time for such concerns. She refused Amy's attempts to make amends.

Richard received a call from Victoria and promptly packed his belongings, making his exit. TJ and the others celebrated at the pub, with Jake joining them later and checking his messages from Amy. She informed him she was working late and would speak with him the following day. Sonya advised Jake against wasting his life on someone who didn't truly see him, revealing that she had overheard Michael's wife confronting Amy about their relationship. Skeptical, Jake stormed out but returned to the hospital. Amy confronted Michael, who admitted he had been waiting for her. They agreed to talk, only to be interrupted by Jake, who found them together.
