In the chamber he shares with Martin, Boyd stumbles upon a ghastly sight - skeletons chained to the walls. Martin, his voice pleading, begs Boyd to end his life. Chaos erupts at the diner, with Donna struggling to maintain a sense of calm amidst the frenzy. Victor and Tabitha seek refuge in a trailer, which Victor has slowly furnished over time with various belongings. Mary, still aggrieved, confronts Kristi over her mysterious absence, accusing her of abandoning her and remaining just two hours away for the past six months.

Brick awakens, finding himself trapped beneath the Matthews' home. Jim and Tom do their utmost to hush his cries, as the situation becomes increasingly dire. As Boyd attempts to liberate Martin from his restraints, a music box begins to tinkle its eerie tune. Martin frantically urges Boyd to flee before the music fades.

Inside the diner, Kenny endeavors to distract Julie from her worries about her father, while two bus passengers who fled the gunshots find themselves stranded at a gas station. Fatima struggles to reunite with other bus passengers who had fled to the post office, but she and Ellis are forced to flee inside as the monsters approach. Shortly after, they hear the ominous sounds of death echoing from the streets.

Jim and Tom continue to soothe Brick, who is now coughing up blood and struggling against the cacophony of sounds. One of the bus passengers, desperate to escape, steals Kenny's gun and holds him at gunpoint. He threatens to leave with Kenny, but Donna remains steadfast. A knock on the window distracts the assailant, allowing Kenny to regain control of the gun, and Donna swiftly brings him down with a shotgun blast.

Boyd persists in his efforts to free Martin, but he pauses when Martin mentions Abby. Just before Martin scratches his arm and presses his bloody wrists against the wound, Boyd notices something creeping beneath his skin. Martin's life ebbs away, and Boyd exits the room to find himself alone in a desolate forest. A dog approaches, and Boyd follows it into the depths of the woods.

At the gas station, the bus passengers hear a knock on the door and cautiously open it. A monster boards the bus and slaughters two passengers hiding within. Boyd manages to evade the monsters through the woods and eventually arrives at the trailer where Victor and Tabitha await. They welcome him inside, but before they can close the door, Elgin appears, pursued by the monsters. Reluctantly, they let him in as well.

Brick's choking noises become more frequent and intense, and it seems as though he has perished. Shortly thereafter, a monster appears and slays Tom. As the sun rises, Julie and the others discover Tom's lifeless body but find Jim still alive. Boyd notices similar creatures wriggling beneath his skin, mirroring Martin's fate. The four survivors from the trailer return to the town, unaware of the horrors that await them.
