Upon arriving at the church, Boyd found it deserted, save for a ballerina gracefully dancing to the tune of a music box. They danced briefly, until worms emerged from her mouth, shattering the illusion.

Suddenly, Boyd awoke in the diner, realizing he had never actually left the church. With a bruised skull and unsteady gait, Kenny escorted him to the clinic. Meanwhile, Ellis and Fatima assisted Donna in loading food onto the truck, as their stocks dwindled. They were entrusted with closing Colony House while Donna hastened to Tian Chen's.

Kristi, searching for a hair tie in Marielle's bag, stumbled upon a packet destined for a treatment center. She confronted Marielle, who confessed to taking some of the liquid morphine. Their argument escalated as Kristi ransacked the medicine cabinet.

As Boyd and Kenny trudged to the clinic, Boyd complained of aching arms and described the worms crawling beneath his skin. He could hear the tinkling of the music box, just before the ballerina emerged from the cellar. Kenny, however, remained oblivious to this macabre spectacle.

At the clinic, Boyd confessed to Kristi about Martin's blood transfusion and the worms it had unleashed. He begged her to end their relationship if his condition worsened. Donna collaborated with Tian Chen and the Matthews to ration the food, but Jim's heart sank as he realized they had only enough supplies for a month.

The music box's melody echoed again, and the ballerina appeared, her grasp tightening around Boyd's throat. When Marielle stumbled upon the scene, Boyd briefly aimed his gun at her.

As Ellis lay dying, Elgin and Fatima set aside their differences to rush him to the clinic. Elgin bravely ventured outside, monsters swarming around the van, and together they loaded Ellis into the vehicle and sped towards the clinic.

Once at the clinic, a united effort was made to alleviate the pressure on Ellis's chest, allowing him to breathe once again. Kristi informed Boyd that a blood transfusion was imperative to save Ellis's life, but Boyd, sharing the same blood type, refused, fearing to spread the unknown malady within his body. Kenny pressed him, offering to be the intermediary, taking Boyd's blood and then transfusing it into Ellis.

Marielle and Kristi observed the worms wriggling beneath Boyd's skin. Finally, Boyd agreed to the transfusion, stepping outside and daring the monsters to emerge. As they did, he slashed his hand and cut the throat of one monster, grasping its neck and infecting it with his blood. The monster fell lifeless, while the others watched with curiosity.

With the blood successfully passed, Kristi prepared to perform the transfusion on Ellis. Donna and Ethan shared a candid conversation in Tian Chen's basement, Ethan confessing his fears of lying to his parents. Donna reassured him that without fear, one would never truly learn the meaning of courage.

Ellis awoke, breathing freely once again. Fatima sought solace with Kristi, breaking down over the thought of losing Ellis. She then requested a pregnancy test. Kenny admonished Boyd for not listening to him earlier, as they both peered out the window at the monster Boyd had infected, wondering if it was truly dead.
