Logan's recovery is promising, and his joy is evident as he's moved out of the ICU. Bailey gifts him his beloved comic book to pass the time, and his university friends swarm around him, expressing their longing for his return to class. However, the joy is short-lived as Logan complains of a perceived leak in his ostomy bag. Schmitt examines him and discovers it's actually fluid from the incision, a common occurrence when skin adheres to the digestive tract. His condition demands a strict diet and a return to the ICU, a serious turn of events that threatens Logan's chances of ever eating normally again.

In the midst of the chaos, paramedics rush in with Gillian Mendelson and Cassandra Lewis, victims of a car accident. Cassandra emerges unscathed, but Gillian sustains injuries. Owen steps forward, his voice firm as he assures Gillian of the utmost care. Little does Aaron, Gillian's husband, know that beneath the surface, Gillian and Cassandra share a secret affair.

During a CT scan, Gillian opens up, her voice trembling as she recounts how she and Cassandra fell into their illicit relationship. She and Aaron had tied the knot after law school and started a family early on. But as the kids grew, Gillian found herself lonely, and the spark in her marriage with Aaron faded. Cassandra entered her life, reigniting the excitement that had once been lost.

When Aaron pressed Gillian to press charges against Cassandra, a startled Gillian blurted out that she had been secretly involved in an affair with Cassandra for several months. She further confessed that shouting oddly alleviated her headaches. During a subsequent CT scan, a startling revelation emerged—Gillian suffered from Arnold-Chiari malformation, a condition often triggered by whiplash. Amelia promptly explained that a minor surgery would suffice to alleviate the pressure in Gillian's brain. Later, Amelia skillfully corrected the malformation, and miraculously, Gillian's headaches vanished.

Meanwhile, in a different wing of the hospital, Simone was attending to Miles Fugerson, a father who sustained a hand injury while rearranging furniture. Miles's wife, Lauren, was pregnant and experiencing regular contractions, but they hoped it was nothing serious. After running some tests, Simone discovered that Lauren was already dilated and labor was imminent.

However, the situation took a frightening turn when Lauren suddenly felt ill, experienced a nosebleed, and then had a severe seizure. Lauren was urgently wheeled into the operating room for delivery, and she delivered a healthy baby. Tragically, however, Lauren began bleeding profusely and developed haemorrhage.

Bailey joined the surgical team and received a briefing from Jo on Lauren's condition. The situation was tenuous for a while, but Bailey and her team managed to stabilize Lauren. Jo and Simone sat down with Miles and delivered the news that while his baby was healthy, Lauren's liver had failed due to preeclampsia. Devastated as he was, Simone urged Miles to remain strong and cling to hope as the curtain fell on Episode 7 of Grey's Anatomy Season 20.
