Episode 5 of "Head Over Heels" unfolds with an unexpected twist as Gyeon-woo accidentally witnesses Sung-ah engaging in a mesmerizing shamanic dance, entwined with Yeom-hwa. His heart shatters into a million pieces, and he retreats, his steps heavy with despair. Unbeknownst to him, as he sits alone, brooding on the street, spirits begin to swirl around him, drawn by his sorrowful aura. Sung-ah, spotting him in his vulnerable state, rushes to his side, her voice trembling as she attempts to apologize for her deception, but Gyeon-woo's ears remain shuttered to her pleas.
Meanwhile, Sung-ah's spiritual mother and her fellow shamans stand sentinel outside a house where an ominous spirit lies imprisoned. The revelation unfolds that the spirit's captivity was but a prelude to an sinister transformation—a ritual meant to elevate it to the status of an evil deity, with the life of the ritualist as the ultimate sacrifice. The shamans fortify the house, ensuring no interruption can halt their grim ceremony. Furthermore, it is disclosed that for the spirit to attain deityhood, it must claim the lives of one hundred souls, a quota it approaches with ominous swiftness.
Elsewhere, Sung-ah reveals her poignant request to Gyeon-woo: five more days of friendship, a poignant plea that hints at deeper, untold stories. She also uncovers the mask behind Yeom-hwa's facade, shedding light on his true nature.
The following day, an eerie tension permeates the air, as everyone senses the strained dynamics between Sung-ah and Gyeon-woo. Finally, Sung-ah escorts Gyeon-woo and Ji-ho to the rooftop, the windswept expanse becoming the stage for her confession. Yet, Gyeon-woo, still nursing his wounds, views her honesty with a jaundiced eye, suspecting her motives and believing her befriending him to be but a cunning maneuver.
Elsewhere still, Yeom-hwa operates in the shadows, orchestrating his subterfuge in secret, preparing to serve the malevolent deity whose emergence looms on the horizon.
Meanwhile, Sung-ah and Ji-ho make a solemn decision to conceal from Gyeon-woo the fact that Ji-ho is aware of Sung-ah's identity as a shaman, driven by their desire to shield him from potential harm. As they venture out for a group study session, chaos ensues when the infant spirit latched onto one of their classmates mysteriously stirs from its slumber. Sung-ah's mind races back to her responsibility to quell the ghost's wails. She endeavors to divert its attention and shield Gyeon-woo, yet her efforts prove futile.
The baby ghost's cries pierce the air, prompting Sung-ah to seize Gyeon-woo's hand, hoping that his presence, akin to a human talisman, would quell the supernatural disturbance. However, the cup he clutches shatters upon impact, leaving his hand gashed. Puzzled by the failure of the human talisman's touch, Sung-ah pays a visit to her classmate's abode seeking clarity. There, she uncovers that the baby ghost had become ensnared within one of the girl's dolls, which the latter refuses to relinquish.
Elsewhere, Gyeon-woo begins to connect the dots, realizing that Sung-ah's every touch bore the hallmarks of a shaman's intent. The following day, amidst the hushed whispers of the library, the baby ghost threatens to resume its wailing. Sung-ah dashes towards Gyeon-woo, instinctively seeking to safeguard him, yet again, the talismanic effect remains elusive. Consequently, the shelves tremble and collapse, injuring Gyeon-woo in the process.
He finds himself confined to a hospital bed but eventually recovers. Sung-ah, her heart heavy with guilt and concern, resolves to dedicate herself solely to his protection. She entrusts Ji-ho with the covert task of concealing a talisman upon Gyeon-woo, hoping to ward off further harm.
Later, Sung-ah crosses paths once more with the deranged woman who had previously tormented her friend, the one haunted by the specter of an infant. The woman, eyes burning with desperation, insists that her baby be returned. This encounter prompts Sung-ah to revisit her friend, relaying the grim revelation that her recent ailments are the work of the vengeful ghost. She warns that unless the haunted doll is surrendered, her friend's life may be forfeited. After a poignant exchange filled with raw emotion, her friend reluctantly concurs.
Elsewhere, Ji-ho notices Gyeon-woo consumed by a torrent of guilt stemming from his former friend's plight following the fiery calamity at their alma mater. Haunted by persistent phone calls filled with accusations, Ji-ho, driven by a desire to shield his friend, volunteers to serve as his human talisman. He even bestows upon Gyeon-woo an action figure, a charm to wield in Ji-ho's absence.
The following day, Sung-ah journeys to return the doll to the grieving mother, who had been pawing through refuse like a desperate scavenger. Amidst the stench and decay, Sung-ah discovers the lip balm she had once gifted to Gyeon-woo, now cast aside and forgotten. Her heart shatters into a million pieces, tears streaming down her cheeks as the heavens too open up, unleashing a torrent of rain. The episode concludes on a poignant note, with Gyeon-woo silently approaching, his umbrella casting a protective shelter over her trembling form.
