The doctors quickly get to work, with Princess handling incoming traumas on the phone, Dana working with Ilana, and clerks sorting through orders on clipboards. There's a separate chart for discharges, and fax machines are brought back into play to communicate between departments and receive scan results and X-rays.
The episode introduces a new patient, Howard Knox, who's brought in with abdominal pain and needs a CT scan to determine the cause. However, his weight of over 450 pounds makes it impossible for him to lie flat on his back. His story is a tragic one, with a car crash, a burn unit visit, and four leg surgeries over ten years leaving him jobless and estranged from his family. The only family member left is Lauren Milford, his estranged sister from Arizona who may be able to help.
Despite his circumstances, Howard is a likable guy who thanks the doctors for their help and tries to lighten the mood with jokes. However, due to his weight, they have no choice but to take him on a road trip to Presbyterian Hospital to use their CT scanner. Jack accompanies them on the journey, with Howard apologizing again for his weight.
The episode showcases the challenges faced by the medical team in dealing with patients like Howard, who require specialized care due to their unique circumstances. It also highlights the importance of communication and teamwork in the face of unexpected challenges.
With an interpreter finally in attendance, Santos rushes back to assess Harlow. Her neck appears to be in considerable pain, possibly due to prolonged laptop use and hunching over. A shot administered to Harlow seems to alleviate the discomfort, and it turns out that she didn't require extensive scans. Santos promptly updates Baran on her patient's condition.
Meanwhile, Roxie's parents arrive to visit their daughter, while Paul continues to push for her transfer to a ward. However, given the current situation they're handling, it may be a while before she can move up. Additionally, Roxie is still experiencing significant pain.
Jackson's case also receives some development in this episode as Nicole Steadman, a woman who runs the parent support group for the psychiatric service, arrives. Nicole's daughter was diagnosed during her sophomore year in college, so she has extensive experience with this situation. Jackson's parents are not pleased when they learn about their son's schizophrenia, especially when Nicole uses her own experiences to help them come to terms with it.
In the main hallway, a new patient named Jackie is wheeled in. She has bitten off half of her tongue while binge-drinking and is covered in blood. She requires stitches, so Langdon steps in to assist. However, Santos is also present, and the two are forced to work together.
Back in the main ER hub, Dana finishes her shift with Ilana and becomes enraged when she discovers another rape kit already sitting in evidence for nearly two weeks. She immediately calls the cops and demands they send someone down to pick them up. It's supposed to be picked up within 72 hours, and the lack of accountability here is not a good look.
Dana also helps steer the chaotic ship by coordinating different charts and bringing everyone back on track. Throughout the ordeal in the ER, there are numerous bumps and issues with the written files, ranging from the wrong pens being used to documents not being filled out properly. It's a chaotic situation, but the doctors do their best with what they have.
In the midst of all this chaos is Mel, who is still torn up about her deposition and a complete bag of nerves. However, she learns that the malpractice case is frivolous and there's no scientific evidence to show they botched anything. It helps her a little, but she's still not out of the woods yet.
Just when things couldn't get any worse, Dana and the other charge nurses learn that 12 patients need medication refills. With recent Medicaid cuts, they are their doctors, but it's absolute chaos as the ER becomes a scrambling mess of doctors trying their best. Dana and Robby look over the carnage and wonder how they're going to get through this day.
