Episode 11 of "Watson" kicks off with Dean Kett, a man plunged into vivid hallucinations, believing himself to be once again amidst the chaos of war. Clad in a hospital gown, his demeanor is a storm of agitation, with the distinct possibility of violent outbursts looming. It falls to Dr. Watson and his team to devise a solution, yet there's a specific reason why Watson is pivotal in this scenario—he was actually by Dean's side on the battlefield, serving as his doctor.

Watson bears a heavy sense of responsibility for Dean's plight, particularly since Dean lost a leg during a Middle Eastern conflict. The situation worsened when soldiers perished and Dean was hit by shrapnel from a suicide bomber on a bicycle.

Elsewhere, Shinwell meets with Moriarty's agent, aware of her involvement in submitting false DNA samples. Shinwell, having done his homework, knows she's being manipulated through her daughter, Ashleigh. He implores her to flee while she still can, offering his assistance. As their conversation inadvertently spills into the main office, where Adam happens to be present, an awkward encounter ensues. The pair manages to defuse the situation by pretending to be lovers, but not before Moriarty's agent slips a can containing real DNA from the desk—setting the stage for her subsequent mission to procure DNA from the team throughout the episode.

Regarding Dean Kett, the team is briefed not only to devise a treatment plan but also to persuade him to accept it. Despite revealing to "Major Watson" about his stomach pain, Dean remains tight-lipped and unforthcoming. Watson decides to bring in Dr. Lubbock, under the guise that she harbors no fondness for Watson and isn't on speaking terms with him. This subterfuge is merely a setup for Watson to silently contribute to Dean's treatment plan from behind the scenes. However, things don't unfold smoothly; Dean manages to slip away unnoticed, escaping from the hospital.

Lubbock and Derian are entrusted with the duty of visiting Dean at his residence amidst the ongoing events. At his place, Lubbock engages in a heartfelt conversation with her friend, lamenting the absence of autonomy in her life and urging Derian to rekindle the spark in his work. While they discreetly search around, they stumble upon a trunk brimming with military paraphernalia. Intriguingly, Dean has meticulously documented all his workout sessions, hinting at a secret endeavor. The reason for this concealment becomes apparent: Dean aims to ace the Army fitness test and re-enlist.

Back at the hospital, Watson and her team overlooked one crucial spot—the roof. Ironically, it's where Dean stands, thwarting a helicopter from landing. Watson delivers an emotional appeal, skillfully convincing Dean to step down from his perilous perch. However, their ordeal is far from over. Dean begins to exhibit symptoms of Ascending Paralysis. It seems his immune system is waging a relentless battle against an unknown malady within his stomach, making every moment crucial.

Dean is consumed by guilt, a weight that stems partly from his earlier oversight—spotting the bomb nestled in the bicycle basket yet failing to sound the alarm or take action. He has unjustly castigated Watson all along, unaware that his true target of blame was his own conscience. As Dean slips into unconsciousness and is hastily intubated, Watson finally pieces together the puzzle. Dean's plight is revealed to be Acute Intermittent Porphyria, a condition where the body's inability to produce adequate heme, a vital constituent of red blood cells, renders it vulnerable. This enigmatic disorder often lies dormant, evading detection until a triggering event unleashes its fury. In Dean's case, his rigorous exercise regimen appears to have set off the attack.

The team administers hemin to Dean, and miraculously, it seems to stabilize him. Upon regaining his senses, Dean makes a profound decision: to forgo re-enlistment and embark on a journey of self-discovery. Moreover, he possesses the contact information for Ramiza, the woman he aided in combat, who coincidentally resides nearby in Missouri. Watson engages in a heartfelt video call with her in the office, catching up on lost time, while Moriarty's agent meets with Shinwell, coldly informing him that his services are no longer required.
