The episode kicks off with Kati scheming to entice Joe (played by Tom Segura) into a romantic encounter with her identical twin sister. He reluctantly agrees, but when the moment of truth arrives, he finds himself unable to perform. Enter Kati's twin, who graciously offers to assist by giving him oral sex. Joe accepts, becomes aroused, and fulfills his promised tryst. As he brings her to climax, he subsequently has intercourse with Kati. During their intimate moments, she playfully instructs him to suck her big toe, only for Kati's twin to chime in, claiming it as her own. Joe, understandably upset, storms out of the scene.

Elsewhere, Carl finds himself accompanying Sarah in the hospital when Lilly Weiland, a representative from The Last Wish Group, enters. Lilly introduces herself and elucidates her organization's mission to grant dying wishes. Carl is stunned to learn that Sarah's final desire is to sleep with Barry, a television personality known for his sandwich advertisements. Carl is beside himself with disbelief, attempting to explain to Barry that Sarah is merely indulging in a fantasy. She compares it to his golfing excursion at the Masters, to which he replies, "That was golf!" Carl becomes increasingly flustered until Barry pulls him aside for a heart-to-heart, to no avail. Soon after, the doctor enters, expressing astonishment that anyone would deny a last wish in his 35-year career and offering to evaluate Carl, whom he deems unnecessary.
In a separate storyline, Tom boards a flight back home to his wife, clutching a necklace as a token of his affection. As he admirers the jewelry, a fellow passenger named Kyle strikes up a conversation, inquiring if Tom is in love. Tom reveals the necklace is for his wife and that he's returning after an extended absence. Recognizing Tom as a renowned comedian, Kyle brings up a controversial joke involving racial and derogatory slurs. Tom clarifies that the joke was about juxtaposing unequal terms and emphasizes that he doesn't use such slurs in real life. Kyle persists, querying whether using one slur isn't as hurtful as another. Tom counters by saying they're two distinct words—one he plays with artistically, the other he avoids entirely. A black man, who happens to be short in stature and standing behind Kyle, chimes in, curious about which words Tom is allowed to use. Tom responds by saying he can use both, leading to a heated argument among the three men about speech and sensitivity.
The dispute is abruptly interrupted when a flight attendant seeks a volunteer for assistance. Tom steps forward and discovers a harrowing truth: both pilots are deceased, victims of a sexual escapade gone wrong. The episode concludes on a cliffhanger, with the flight attendant informing Tom that he must now land the plane.
