The opening episode of Breathless Season 2 picks up immediately after the hospital blackout of the previous season, as May calls the police on Rocio for kidnapping their child. Rocio, claiming she just wanted to keep their daughter from being alone during the chaos, is found by the authorities. Meanwhile, Pilar orders Biel to assist in the operation to save bleeding Jesica. As Lluis receives the news that Sorolla is being privatized immediately, Dr. Moa realizes this is Patricia's doing and is filled with anger.

The scene shifts two weeks later, where Jaume expresses his unwillingness to be Sorolla's manager anymore, and Patricia selects Nicolas instead. Biel is the only one visibly upset, while Nicolas tries to win everyone over by insisting he won't make rash decisions. To prove his worth, he brings in Sophie Lafont, one of Europe's top oncologists, to treat Patricia. Moa feels betrayed, as expected.

Meanwhile, Jesica is improving and Moa is impressed with Biel's work, even offering him a double residency in oncology and general surgery. As Biel checks on Jesica, she tries to have a moment with him before her boyfriend, Lluis, cordially thanks and dismisses him.

Oscar seeks help from Dr. Neus to overcome his addiction, and Neus suggests avoiding people who do drugs, including Quique. Oscar is conflicted as he has grown to care for Quique. When he meets him to break up, he is shocked when Quique breaks up with him first, saying they were just having fun. Oscar is upset that Quique doesn't return his feelings.

May and Rocio continue their usual arguments as their daughter falls ill. Nicolas is amused by the drama but clearly unhappy that they are bringing it to the hospital. May worries that she is not being a good mother, and Rocio tries to comfort her but ends up using it as an opportunity to claim that taking their daughter to Germany may take a toll on her. May is torn between her worries and the belief from Biel and Quique that having friends around who can help if the baby gets sick again is better.

Elsewhere, Leo encounters a pregnant patient who is lashing out at Dr. Murillo for coercing her into labor. Despite Leo's admonishment for compassion, Murillo remains indifferent. When the woman eventually goes into labor, Leo is horrified to witness Murillo employing a painful maneuver, disregarding her previous c-section history. The patient starts bleeding, and Leo forcibly pushes Murillo aside to take over.

Meanwhile, Moa introduces himself to Sophia, attempting to strike a rapport with her. However, she is uninterested in friendships, appearing nonchalant and solely focused on her work. Moa then turns his attention to Patricia, calling her out for corruption and abusing her power to receive preferential treatment. He does the same to Nicolas when the manager advises him to overlook moral dilemmas for the sake of the department. Frustrated, Moa publicly expresses his disapproval of Patricia, leading to a vote to oust her due to the articles detailing her corruption. In retaliation, Patricia announces an election in three months to select Valencia's president. Moa points out that she cannot campaign in her condition, but she promptly experiences an attack that Moa considers a scan necessary for. Sophia believes it was merely an anxiety attack, but Patricia trusts Moa's judgement.

Meanwhile, Jesica continues to suffer in pain, with both Pilar and Nurse Blanca convinced that she is recovering as expected. That night, Jesica collapses only to find Biel sleeping in her room. He tries to check on her, but she firmly tells him to put their love triangle drama on hold until she recovers. Alone, she quietly increases her dosage of painkillers.

Pilar returns home to find it in a state of chaos. Oscar insists he has not taken anything but is in a manic state, throwing around the furniture. Once he comes to his senses, Oscar is terrified of himself, and Neus believes his manic episode is a symptom of bipolar disorder.

As for Biel, he is confused and confides in Quique about Jesica's erratic behavior. Quique believes it is due to the painkillers. Nicolas tries to talk to Biel, who gives him the cold shoulder. Quique is puzzled until Biel reveals that he is Nicolas' son. They have a strained relationship as Nicolas was never there for him.

Patricia's scan reveals a new clot causing her respiratory attack. After the surgery, Sophia reveals that it was a side effect of her treatment, and they cannot proceed further, which means there is nothing more they can do. Patricia begs Moa for help but knows this is it for her, and she cries helplessly.

As the pregnant patient's concerns were voiced, Nicolas turned to Leo for his insights, and Leo echoed the patient's sentiments. With a determination to revitalize the gynecology department, Nicolas vowed to do his utmost. In a hushed conversation with Patricia, Sophia, seemingly unkindly, but in truth seeking funding from Sorolla's involvement, hinted at a new, yet-to-be-approved trial. She wove a tale of historical significance, luring Patricia with the promise of making a mark in medical history—all while setting a condition: Moa's removal as head of oncology. Nicolas's enthusiasm spilled over as he shared with Moa that he was already in the midst of fulfilling his promise to revamp the department.

Meanwhile, Jesica believed there was something within her that needed to be released, prompting her to cut herself open. Lluis, who stumbled upon her bleeding profusely, went to inform her that her assailant had been apprehended. During the surgery, Pilar realized that toxins had reached Jesica's brain, causing hallucinations and a clot in her liver. Pilar was at a loss for words, while Biel implored her to act swiftly.
