As the second episode of Breathless Season 2 unfurls, we find Jesica waking up to the harrowing news that she has just 36 hours to secure a liver transplant. The urgency of her situation is mirrored by Nicolas's decision to assign two healthcare workers to each patient, a measure aimed at shielding doctors from potential attacks. However, this move creates discontent among the staff who feel they require more doctors, nurses, and equipment.

May, re-instated to her duties, endeavors to be cordial with Rocia, as she takes on the case of Ester, an 18-year-old Jehovah's Witness who is hemorrhaging and in dire need of surgery. Despite May's persuasive efforts, Ester's parents arrive and halt her progress, revealing that they were not contacted by Ester herself, implying hospital intervention.
Sophie, preoccupied with her trial, skips the department meetings but shares new rules that include admitting 30% more patients despite the oncology staff's lack of equipment. Moa is disheartened when he notices she has been assigned a state-of-the-art, heavily fortified floor.
As Jesica's desperation mounts, Biel pleads with the parents of a deceased patient to consider organ donation. His effort is met with rejection, and Quique warns him against jeopardizing his career. Meanwhile, Quique embarks on a personal adventure with Jon, a plastic surgeon, who flirts and invites him to join an under-the-table surgery, promising half the earnings to the staff involved. While initially tempted, Quique's concerns about lack of supervision mount until Jon dismisses the surgery as trivial and coaxes Quique into participating regardless.
May's disappointment with Ester and families who reject their children is palpable, reminding her of her own parents' rejection. Rocia offers solace by revealing that her own parents accept her daughter and have a place for May if she needs it. The episode concludes with a sense of hope and resilience amidst the chaos and turmoil of the hospital.
Oscar, beaming with hope, arrives at the hospital eager to be "cured" of his bipolar disorder. Anxious, Pilar attempts to lecture Neus, but the therapist implores her to simply offer unwavering support to Oscar. Despite her doubts, Pilar does her utmost.
Jesica's condition worsens, and her life hangs in the balance without a donor in the next few hours. Biel, in a desperate bid, conducts a secret test on the deceased patient. Lluis catches him in the act, revealing that he risked everything only to find that the donor was incompatible. He warns Biel against crossing the line.
Alfonso, the party leader, has agreed to back Patricia's early re-election campaign on the promise that she has been cured. She seeks Moa's approval for the trial, but he is opposed. Sophie, however, figures it out and rebukes Moa for misguiding Patricia. She believes that Patricia should not waste her short life in hospitals. Frantic, Sophie reveals the importance of her trial - it can detect breast cancer much earlier. But Moa accuses her of gatekeeping instead of trusting the department.
Pilar reveals that there is a donor available, but Jesica was not chosen by one point. Frustrated, Lluis also takes the illegal route by manipulating her test results. Jesica is selected, and the liver transplant is a success. As they wait for her to wake up, Lluis requests that Biel leave him and Jesica alone.
May finds Ester bleeding profusely and promises to operate without transfusions, which Ester agrees to. However, during the surgery, she starts bleeding out. Rocio wakes Ester up and obtains her consent for a transfusion. However, Ester's parents find out and disown her. May and Rocio discover that the nurse who conducted the surgery is a Jehovah's Witness and he is arrested for disclosing patient information.
Moa confronts Patricia, who has second thoughts but then receives a sudden boost from Moa to keep fighting. He believes that Sophie will be successful, and she agrees. During her treatment, she befriends Ximo, a male breast cancer patient.
May is relieved to have Rocio by her side. They agree to live together for a while but Germany is still an option on the table. When Nurse Blanca catches Rocio alone, she wonders if May knows that Rocio was actually leaving with their daughter in a taxi during the blackout.
Sophie joins the next debrief meeting, apologizing for ignoring the department. But Moa's smile falters when she chooses Biel to join her in her clinical trial.
At the end of the episode, Nicolas fires Lluis. There will be an investigation into Jesica's fabricated test results. Nicolas hints that he is not above illegal dealings but he is never caught.
