In the third episode of Breathless Season 2, set one month later, Quique finds himself in his 15th private surgery with Jon. As usual, Jon’s nonchalant attitude prevails, with music blaring and chatter filling the air. However, Cristobal's vital signs start to plummet, and when nothing seems to work, everyone scurries off with Jon simply stitching Cristobal up. May and Biel fret over Quique's predicament, knowing that it's also illegal for a resident to operate without supervision.

Meanwhile, Lluis has been tending to Jesica and they're in a comfortable place. Upon their return to the hospital, Jesica comforts Lluis, convinced that Nicolas was bluffing about the investigation. Now an ordinary doctor, Lluis is sent to the ER while Jesica persuades Pilar to grant her a surgery.
Nicolas has kept his promise to Leo and overhauled the gynac department. Patricia brings reporters to paint herself in a positive light, taking credit for the changes. The ER staff are frustrated with their understaffing, and under the guise of a revamp, Nicolas claims they'll send non-critical patients to other hospitals.
Cristobal's family is suing, and Pilar bans Jon from using the OR. Quique gets a strike and is sent to the ER. One more strike means he's kicked out of the hospital. He's upset, expecting Jon to back him up, but Jon is nonchalant, claiming he'll just find another hospital. Pilar isn't done yet as she confronts Nicolas, pointing out that the cost benefits don't outweigh the risks. In response, Nicolas offers her the director's position, claiming he needs a partner who isn't a pushover. She's shocked.
Patricia accepts her fate and shaves off her hair. Ximo is bubbly and chatty, but his symptoms are worse than hers. Worried, Patricia wants an explanation, but as usual, Sophie is vague. Moa checks on Patricia who rants about Sophie's nonchalant nature. Moa comforts her and they end up apologizing and planning a dinner date.
Biel tries to visit Jesica but she draws the line for good, choosing Lluis instead. Upset, he storms off. She has bigger problems as she experiences PTSD during her first surgery back. She runs off and Lluis tries to help. She insists she'll be fine but no one can know as she'll be put on leave.
It's date night, but Patricia can't shake the anxiety that plagues her over her results. For reassurance, Moa undergoes a CAT scan, and the good news echoes through the air—most of the tumor is gone. She will live! Patricia's tears of joy flow freely, and she nearly plants a kiss on Moa.
Meanwhile, on another shift, Jesica turns to self-help books for solace. But a major accident in the ER sends her into a tailspin of hyperventilation. Lluis is at a loss, while Rocio is overwhelmed—she doesn't have time to babysit him. However, he quickly recalls his composure.
Jon arrives to apologize to Cristobal's wife in an attempt to evade the lawsuit, but Cristobal goes into shock. The wife insists he doesn't belong in the OR, but he barges in and saves Cristobal. He then apologizes to Quique, revealing he took responsibility and got the lawsuit and strike dropped. Quique is elated, but May warns him against falling for Jon's charms.
Sophie is dismayed that Patricia underwent a CAT scan on her own. She's also worried about her research being stolen, as Patricia is filming a video about her success. When Patricia refuses to cancel the video, Sophie enlists Alfonso to call and shut it down.
A young and healthy pregnant woman suddenly experiences pain. The fetus has a tumor, but she refuses an abortion, leaving surgery as the only option. Biel is shocked to learn she's carrying Nicolas' child. When he confronts his father, Nicolas claims he's trying to be better. Biel is upset, and we learn that Nicolas abandoned him when he had cancer. Moa checks on Biel, but he assures her he won't let his feelings cloud the surgery.
While waiting for news, Pilar sits with Nicolas. He admits he was a coward but wants to fix things with Biel. She sees his sincerity and accepts his role as director. Soon, Biel breaks the news that mother and child are safe and out of relief, Nicolas hugs him. Biel is in a daze and walks off.
Blanca and Rocio finally get a break and are exhausted. To pep them up, Blanca suggests lorazepam, an anxiety pill. Overstimulated by the day's events, Rocio takes it without hesitation.
As for Jesica, she tries another surgery but once again hyperventilates and backs out. She decides to take on administrative work, but Pilar suddenly reveals she wants Jesica to take up her place as head of surgery—a bold move that leaves Jesica both surprised and grateful for the opportunity.
