The fourth episode of Breathless Season 2 kicks off with a lively pool party hosted by Patricia. As Ximo suddenly fades into unconsciousness, Moa springs into action, rushing him to the ER. The news that Sophie has informed Ximo of his tumor's reduction only adds to the chaos. Moa, however, finds herself locked out of his files and evaded when she arrives. A nagging sense of unease settles in Patricia's heart, prompting her to worry that Sophie's claims of success might be more than just a well-crafted facade. Moa, determined to uncover the truth, agrees to delve deeper.

Meanwhile, Rocio finds herself in the grip of a vicious cycle, reliant on anxiety pills to cope with the mounting stress. The ER is further strained by a Covid outbreak, and Pilar voices her disapproval of Nicolas's rigid protocols. Stunned by Pilar's concern, she suggests diverting budget funds from the gynac department to aid the ER. Nicolas, however, refuses, explaining that the government funds them for increasing birth rates.
Elsewhere, Jesica watches with a tender heart as Biel forms a bond with Robbie, a young boy battling cancer. With a nurturing touch, Biel encourages him to keep fighting and even convinces him to text the girl he admires. Robbie's fear of her response leaves him anxious, and Biel promises to check on his behalf once his surgery is done.
Pilar has brought in Jon to assist with a patient's amputation. Quique is impressed but keeps his distance, while Jon persists in his flirtatious attempts to win back his trust. He even goes so far as to invite Quique to the surgery itself.
As head of surgery, Jesica purposely avoids taking on any surgeries, only for Pilar to push her into taking one. Meanwhile, Hugo struggles with depression and his mother Leo feels the weight of her guilt when Lluis reproaches her for abandoning their son in the first place.
Rocio is a shell of her former self, and in a moment of frustration, she lashes out at May when she asks for more help with Africa, their daughter. She vows to be present at Africa's next doctor's appointment.
Moa's investigation reveals that Sophie is frequently switching hospitals and her success rate might be lower than she claims. As she drives home, Jesica encounters a pileup accident and despite her initial panic, she springs into action. By the time the paramedics arrive, she has already saved a girl's leg, her quick thinking and skill evident in the chaos.
Robbie's surgery turned into a tragic ordeal, with no viable options left for him. Moa pleaded with Biel to deliver the devastating news, believing it would hold significance for Robbie's soul. Biel's heart was heavy, and Nicolas saw an opportunity to mend fences. Yet, Biel's resentment lingered, refusing to forgive him for his abandonment.
Jon's unwavering attention towards Quique baffled the latter, until an unexpected kiss sealed their deal for a drink. As they were about to depart, a woman and her daughter appeared, claiming to be Jon's family. Quique was taken aback but was promptly summoned to the Emergency Room (ER).
The influx of patients from the pileup overwhelmed the ER, forcing Pilar to enforce a cap on its capacity. Jesica bravely operated on a patient in the ambulance due to the second hospital's inaccessibility. Despite the grueling challenge, she soon regained her composure and saved the patient's life. Her relief at returning to surgery was palpable, and Lluis's joy for her was evident. Seeing his genuine concern, she accepted his proposal of marriage.
Biel asked Sophie for assistance in the ER but was met with a firm refusal. Their conversation was interrupted by the news of Ximo's death. Moa's anger boiled over, accusing Sophie of deceit, but Patricia intervened, calming Moa's fury. Biel was disturbed, while Sophie reassured him that their efforts would eventually bring a cure for cancer.
Rocio's missed doctor's appointment was a foregone conclusion, and May made the decision to leave. Their argument turned emotional as May broke down, realizing she couldn't raise a child alone. Rocio promised to be there for her, and their kiss symbolized their commitment to each other.
Seeking answers, Moa stole Biel's keycard and accessed all of Sophie's case files. Both Moa and Patricia were shocked to discover that the scans labeled Ximo did not belong to him.
