The fifth episode of Breathless Season 2 commences with a bold investigation by Patricia and Moa into the enigmatic Sophie. As the truth about Moa's sneaky acquisition of Biel's keycard unfolds, Biel confronts her with a mix of anger and surprise. Moa, in a bid for transparency, reveals all, and together they confront Sophie head-on. Sophie, with a deft touch of manipulation, claims to have misplaced Ximo's vital scans, but Moa dares to call her bluff, threatening to report her to the authorities. Patricia, wary of Sophie's charms, is torn between fear and a desire for a clean break. Sophie, with her masterful gaslighting skills, convinces Patricia that they are embarking on a noble endeavor and resumes the treatment, unaware of the damage she has caused.

Meanwhile, Rocio's addiction spirals out of control as she finds herself working in the ER even when off-duty. Her memory starts to slip, and she rashly discards her pills. Blanca notices her behavior but Rocio is evasive, hiding behind her addiction's veil.
Lluis announces his engagement to the entire ward, leaving Biel hurt and Jesica trying to make amends. With the help of Nicolas, who is eager to mend fences with Biel, they hang out over snacks, trying to salvage their friendship.
The deal to fund the ER through Healock Pharma seems too good to be true – except for one catch: they want to become Sorolla Hospital's exclusive supplier. Pilar is displeased, and Nicolas is forced to delay the agreement, causing tension in the hospital's corridors.
Jon tries to explain his situation, revealing his divorce and his growing affection for Quique. Quique is unmoved by his words and draws a line in the sand. His concerns deepen when he realizes several patients have developed an infection – all of them operated in OR 5. He shares this information with Pilar, but she insists their OR is not contaminated as they employ the best decontamination service.
Lluis and Leo deal with an assault victim named Eva, whose dark secrets come to light when she tells Dr. Neus that she deserves the attack because she accidentally killed her little brothers due to her addiction. Eva is left alone by her parents' disownment and seeks solace in her own misery. Meanwhile, Leo learns from Neus that Lluis is engaged and not inviting her to the wedding out of consideration for Hugo's feelings.
As Jon stumbles upon Quique trying to obtain a sample from OR 5, he decides to lend a hand. Quique apologizes for his harshness, citing his past with exes and his fear of repeating the same mistakes. In a twist of fate, Quique discovers Oscar having an episode, thinking he's using but Oscar protests that he's not an addict like Quique.
In the midst of it all, Moa reaches out to journalist Marcos, hoping to delve deeper into Sophie's secrets. Patricia, on the other hand, is consumed by fear and finds it difficult to maintain her focus on her campaign. Emilio warns her against meddling with Sophie, reminding her that she brought her to Sorolla's attention in the first place. If Sophie is exposed, so is Patricia.
Leo sits by Eva's side, insisting that she made a mistake and needs to move on. Eva suddenly experiences a sudden pain and passes away. Her family refuses to take her body, leaving Leo deeply affected and determined not to let her son feel the same way about her. Leo can't understand why she was so angry when the rape victim and the system forgave Hugo. Neus advises her that it's not too late to repair things with Hugo.
Sophie takes it upon herself to scan Ximo's corpse as a way to prove herself. She blames Moa for pushing her to such extremes, unaware that the scan reveals no tumour. Moa is skeptical but Patricia believes her and calls him out for being stubborn.
Jon and Quique confirm that the operating room (OR) is contaminated and inform Pilar of the situation. She is furious to learn that Nicolas has outsourced the OR cleanings to cut costs. Unfortunately, there's an ongoing surgery in OR 5, leaving the staff terrified and hesitant to continue. With the help of the lead surgeon, Pilar, Quique, and Jon manage to complete the surgery.
Rocio, overwhelmed in the emergency room (ER), abandons a patient and has a panic attack. May helps her through it but Rocio is still upset, blaming herself for everything that goes wrong. May promises not to leave her side despite it all.
Taking Neus' advice, Leo wants to see Hugo, much to Lluis's delight. Meanwhile, Moa is about to drop the investigation when Marcos reveals that most of Sophie's patients have died, adding fuel to the fire.
