In the opening scene of Malcolm in the Middle: Life’s Still Unfair, Episode 3, Malcolm approaches Leah with a request to mediate with Tristan on his behalf. Leah informs him that Tristan remains unforgiving, and she urges him to attend the anniversary party as a means of reconciling with his family, a suggestion she had previously made. Malcolm's imagination conjures up a disastrous scenario of reconnecting with his family, which prompts him to reconsider his attendance.

Meanwhile, Hal is found in the meditation room, under the influence of drugs, experiencing hallucinations and an inability to calm down. He finds himself in an out-of-body state as he engages in a self-conversation.
Elsewhere, Reese stages a ransacking of Kelly's school principal's office, aiming to frame them for staging a protest. When Kelly confronts him about it, Reese reveals that he did it as retaliation for exposing his secret to the family.
At the same time, Francis shares with Piama that he wants to take action to make their pregnancy news more of a priority for Lois. However, Piama expresses frustration at Francis's focus on Lois rather than their own situation. Despite her frustration, Francis continues to brainstorm ways to capture Lois's attention.
Leah is in her room on a video call with Jason, where they begin flirting. Malcolm interrupts to speak with her and expresses his desire to move on from Tristan since she refuses to talk to him. He adds that he now wants to focus more on Leah, which makes her uncomfortable.
Kelly attempts to get back at Reese by reporting his unreported income to the IRS. Reese accepts the challenge and decides to escalate the situation further.
Lois endeavors to reach out to Hal but finds herself unable to connect, prompting her to call upon Abe for assistance. Abe makes an excuse and promises to bring Hal to the party. In the tranquil meditation room, Hal experiences a hallucination of a conversation with his alter ego. He reflects on living in perpetual fear and admits that with his children moving on, he feels no longer indispensable. His alter ego urges him to start prioritizing himself.
Back at the house, Francis offers Lois a $1000 cheque to help her and attempts to express his feelings, but she brushes him off. Later, Francis notices a pair of baby shoes in her shopping bag and is touched, teasing her about it.
Elsewhere, Tristan and Leah coerce Malcolm into the car and drive him to the anniversary party. Meanwhile, Hal continues to struggle with his hallucinations as he tries to make sense of himself. At the same time, Lois becomes increasingly anxious as the party plans begin to unravel.
In the car, Malcolm tries to convince Leah and Tristan to turn back. He claims he is trying to protect Leah from his family, but Leah argues that by hiding the truth, he is depriving her of her right to choose. Malcolm realizes his mistake and begins to calm down.
Tristan uses this moment to reveal her secret. She tells Malcolm that she killed her stepfather, who had been abusive to her mother, and admits that she hid it because of his political views.
Meanwhile, Lois discovers Hal in a drugged state near a dumpster and picks him up. She drives in a state of panic while still trying to manage last-minute party arrangements. The episode ends with Lois and Hal getting into a car accident.
