In the fourth episode of Man on Fire, the extraction team awaits Poe and Creasy, but they never arrive. Creasy tricks Mateus, the kid, and traps him, his mother, and Osmar in the panic room. After securing their confinement, he makes his ultimatum: Osmar, aka Gabriel, is a dead man, but Creasy will spare the family if he divulges everything about the FRP and the bombing.

Meanwhile, in the favela, Melo alerts her gang with car blinkers that Osmar's thugs are holding her at gunpoint. Everyone arms themselves immediately, leaving Poe confused and thinking the favela is after her. Vico takes control and rants about handling Beto's assignment. The favela starts shooting, killing one of the thugs. Gray Beard, the other thug, makes a run for it but is captured by Poe and Livro.

Osmar is cooperative, and Creasy uploads all blueprints to his old CIA portal. Tappen decides to analyze it before sharing it with President Carmo. During the interrogation, Osmar reveals that someone was going to track Poe in the USA if he failed to capture her in Rio.

Gray Beard calls Osmar's home and demands a hostage swap - Poe and Livro for Osmar and his family. On the call, Poe begs Creasy for help, triggering a memory of his team getting killed. He passes out, and Osmar coaches his son to move the swivel chair to the panic room exit.

Vico is furious with Beto for putting Livro at risk for money. While Beto and Melo try to contact Creasy for hostage negotiations, Vico takes matters into his own hands to save Livro. Gray Beard holds Poe in front of him, and Vico plans to kill Poe, allowing his right-hand man to kill Gray Beard. However, Gray Beard keeps moving, and Vico accidentally shoots Livro in the arm. At the same time, Creasy wakes up and prevents Mateus from escaping. Gray Beard calls the panic room again and threatens to kill the two teens unless Vico stops shooting at him.

We see that Creasy has also uploaded a recording of Osmar listing everyone involved in the bombing. Tappen and the CIA director are impressed and finally decide to share it all with Soares.

Melo is able to contact Creasy who agrees to a hostage exchange. Duda, Vico's superior, is upset that he jeopardized the entire assignment and put Livro at risk.

The hostage swap unfolds amidst the rugged mountains, where Osmar's well-trained men maintain a cautious distance, while Creasy holds the family at bay. The favela members, jittery and unseasoned, train their guns on Gray Beard, who has his firearm pointed at the two teenagers. It's a tense moment, but the swap succeeds nonetheless. Beto scolds Livro for causing a mess, and Poe weeps, blaming Creasy for abandoning her. He apologizes to her, but the crisis isn't over yet; Poe knows the biker's identity. Creasy, Poe, and Melo pick up Melo's daughter, Mariana, and they head to another safehouse.

News reports indicate that the Brazilian authorities have identified and neutralized all those involved in the bombing. Osmar has allegedly taken his own life, and his family has apparently been arrested while attempting to flee the city.

Creasy remains radio silent, and Tappen announces that he will personally go to Rio to assist him. But alone, he confides to someone that he will kill Creasy and Poe.
